The Adoption Coordinator will provide case management, adoption services and post adoption services to assigned regions.
Principal Duties and Responsibilities
Complete all preservice training in required time frames. Staff will annually complete 24 hours of training.
Work in conjunction with area Recruiter/Trainers in recruiting and screening foster-to-adopt families.
Once a foster-to-adopt family is licensed, the Adoption Coordinator will do specialized pre-adoption training with them. This training will include the history of adoption, the adoption process, costs of adoption, common adoption issues and support services available to all families.
Conduct a thorough pre-placement assessment for all families identified for adoption, and complete them following closely all NC DHHS guidelines.
Serve as the liaison between the family and the Department of Social Services.
Work closely with foster care staff to ensure quality matching and placement of children with foster-to-adopt families.
Coordinate with the foster care case manager to provide regular case consultation once a family has been chosen to adopt by the county DSS Adoption Committee.
Demonstrate effective use of the Teaching Family Model in consultation work with foster-to-adopt families.
Will provide phone consultation and in-person first responder crisis support with foster-to-adopt families as needed and shared with other foster care staff.
Responsible for providing adoption preparation support and consultation for all families and child and family team members during the adoption process.
Responsible for completing all legal paperwork, in coordination with the county DSS.
Responsible for providing quarterly contacts for one year post adoption finalization.
Complete consultation documentation, monthly census, and billing paperwork to Program Manager within agency guidelines.
Collect all child related documentation monthly and submit to Program Manger within agency guidelines.
Work with CHA fund raising staff on any events that may highlight our adoption program.
Attend state-wide meetings as directed by the Program Manager.
Requirements:Bachelor's degree in psychology, social work, or related field from a four-year college or university required
Two years of previous work experience in a related field required.
Qualified Professional (QP) designation required
Prefer extensive knowledge of issues related to adoptive family identification, strengthening family bonding and claiming, legal issues in adoption as well as post adoption support.
Experience training and supporting foster families is preferred but not required.
