GENERAL SUMMARY
According to established procedures, using principles specific to pediatrics, adolescent, adults, and elderly patients, secures and obtains factual information relative to patients’ social, financial and limited medical history; enters information into hospital computer.
ESSENTIAL JOB FUNCTIONS
1. Interviews incoming patients or his designee, records identifying information required into hospital registration system. Obtains necessary consents forms.
2. Admits patients as inpatient/observation admissions via ER or as direct admits from the physician office. Assigns patient to a room. Compiles a chart, prepares plate, and arm band which accompanies the patient to the floor. Calls insurance company to acquire precertification number for admission. Prepares an insurance folder consisting of a copy of the admission, insurance forms that were filled out and signed and a copy of the insurance card. Sends completed folder to the business office. Collects valuables. Escorts patient to room. In the event the patient has no insurance, all clerks will make reasonable efforts to obtain demographic information from these patients which includes: calling the patient’s family, visiting the patient/family in the room, calling the nursing home (if patient is a resident), reviewing the paramedic run sheet and searching the patient’s belongings.
3. Registers patients for ambulatory surgeries. Calls patients in advance for pre-admission testing, providing ambulatory surgery instructions. Pre-admits patients for procedures. Calls insurance company to verify active coverage and pre-certify patient, if necessary.
4. Provides backup phone coverage to the switchboard for lunch and between the hours of 11:00 p.m. – 7:00 a.m., 7 days per week.
5. Registers patients for outpatient services, gathering demographic and insurance information. Obtains authorization for services when needed via a referral form or by phone from the physician. Enters orders into the order entry portion of the system. Obtains all necessary signatures.
6. Discharge and transfer patient via the hospital computer when requested.
7. Collects payments for phone connection, clinical services and co-payments. Balances bank and makes cash/credit card deposits daily with the cashier.
8. Verifies information when patient expires. Obtains signatures for consent for autopsy and permit for removal; type death certificates; notify coroner; call public administrator when necessary.
9. Communicates with family regarding patient’s condition.
10. Makes referrals to appropriate social service agencies.
11. Monitors lobby area and Quiet Room for cleanliness.
12. Observes waiting patient’s condition and report to Triage, if necessary.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
1. Work requires a high school diploma plus 1-2 years college. Must have the ability to secure information from patients and deal effectively with patients, employees and external contacts.
2. Must be familiar with hospital computer systems, or similar system, specifically in the registration and/or patient account applications.
3. Exceptional interpersonal skills, tact and understanding necessary to interact with patients of all ages and medical personnel who treat and/or admit patients.
4. Able to use the following equipment: Computer, embosser, cash register.
5. Focus on delivering exceptional customer service and patient care.
6. Exceptional communication skills.
