Description

Job Summary: The ADAP and Access Coordinator provides program support as a member of the multi-disciplinary HIV care team at Santa Rosa Community Health (SRCH), application assistance with ADAP and other Office of AIDS programs, documenting Ryan White eligibility/Cap on Charges, and ensuring patients’ insurance/coverage and non-medical social needs are addressed. These duties will primarily occur on-site at SRCH; however, patient enrollment activities may rarely occur at other locations such as local hospitals.

Pay Range: $26.82 – $36.74/hour

Specific Tasks/Duties:

  • Screen VIDA and at-risk patients for eligibility for all relevant insurance, coverage and programs including ADAP, PrEP-AP, OA-HIPP, OA-PCIP, Lark Drugs Prescription Share of Cost Program, Medi-Cal, Ryan White and Covered CA.
  • Identify, track and assist patients with annual and semi-annual enrollments and renewals of the programs listed in the prior bullet point to prevent lapses in coverage, including scheduling appointments and ensuring patients are aware of necessary documentation.
  • Track applications that have been submitted and follow up with patients and/or appropriate agencies as necessary; provide support/education to patients if any issues arise with their applications.
  • Process patient ADAP complaints and grievances according to CA Office of AIDS and clinic policies.
  • Ensure all SRCH information in the AES is accurate and current.
  • Provide consultation and support to Access Navigators processing ADAP/PrEP-AP at all SRCH sites if the case arises that a patient does not wish to enroll/re-certify at VIDA's clinic campus.
  • Keep a log of patients who are assisted both in person and by phone (daily, weekly, and monthly statistics).
  • Help patients with private insurance navigate the insurance process including acting as the main point of contact with private insurers to procure Prior Authorizations.
  • Support the facilitation of English/Spanish consumer advisory focus groups.
  • Schedule appointments, track, interface, and troubleshoot lab orders/results, perform pre-registration and confirmation calls for Team VIDA as assigned.
  • Route triage calls to appropriate RN or other VIDA team members as appropriate.
  • Assist VIDA patients in need of medical transportation to appointments and dental referrals.

 

Other Duties

  • Attend staff meetings and participate in team-based Quality Improvement processes.
  • Review accuracy of client data, maintain up-to-date client-related records, complete data entry into EHR and appropriate database(s) and meet deadlines for required documentation according to the protocols/standards of SRCH and Team VIDA.
  • Maintain client records in accordance with protocol; maintain patient confidentiality at all times.
  • Maintain appropriate professional boundaries at all times with all clients.
  • Participate in ongoing training, workshops, conferences, and forums to increase knowledge as directed by supervisor.
  • Other duties as assigned.
Qualifications

MINIMUM QUALIFICATIONS:

Education:

  • High school diploma or equivalent.
  • Bachelor's degree preferred.
Certifications/Trainings:
  • CDPH Office of AIDS ADAP/PreP-AP Certification required
  • Must have the ability to successfully complete the Certified Application Assistant program within first 30 days in the position.
  • Must have ability to successfully complete training, pass the exam, and pass the Department of Justice screening to become a Certified Enrollment Counselor with Covered California.
  • Must yearly take the Certified Enrollment Counselor training and pass the exam with Covered California.

Experience:

  • Bilingual in English and Spanish (speak, read, and write) Preferred.
  • Experience with HIV disease preferred.
  • Experience with Medi-Cal and ADAP preferred.
  • Experience with electronic health records preferred.
  • Must have valid California driver's license

Knowledge and Skills:

  • Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
  • Ability to comprehend complex program requirements and educate patients on those requirements with simple language.
  • Maintains confidentiality of patients at all times.
  • Proficiency with Microsoft Office applications including Outlook, Word, Excel.
  • Ability to work independently and as part of a team.
  • Sensitivity to and willingness to interact with persons of various social, cultural, economic, and educational backgrounds with topics including sensitive health issues.
  • Strong verbal, written, and interpersonal communication.
  • Ability to maintain detailed logs of activity and status of insurance application.
  • Demonstrated ability to type 35 words per minute.

Physical Requirements:

While performing the duties of this job, this position is frequently required to do the following:
