Duties and Responsibilities:
- Coordinates and schedules all dental, medical and transportation appointments for the MCC Clinic.
- Coordinates patient dental appointments with the current Dental Provider.
- Submits and tracks dental referrals.
- Mails patient dental appointment cards.
- Monitors patient dental attendance.
- Assists patients with establishing primary care appointments for patient who choose a primary care provider at Tandem Health.
- Mails patient appointment letters.
- Provides patient reminder calls and assesses patient transportation needs.
- Schedules follow-up appointments for patients before they leave.
- Provides follow-up on all broken appointments to minimize patients falling through the cracks and becoming lost to care.
- Maintains a ready-call list and fills broken and cancelled appointments to maintain clinic flow.
- Monitors clinic flow.
- Provides treatment adherence counseling to RW ADAP recipients.
- Monitors appointment/medication adherence via the following encounter methods: face-to-face, home visits, pharmacy calls and/or telephone.
- Tracks patient medication clinic deliveries.
- Assists with completing/submitting patient ADAP recertification forms as needed.
- Coordinates transportation with the Patient Transporter.
- Provides indirect supervision to the Patient Transporter.
- Resolves all patient related transportation issues.
- Maintains patient dental, medical, treatment adherence and transportation usage via the Provide Enterprise (PE) data system.
- Performs other duties as assigned by supervisor.
Qualifications:
High School Diploma or GED required. A minimum of 4 years of related work experience or a Bachelor's degree in a related field, or an equivalent combination of education and work experience. Must be flexible as to the changing needs of the center. Must be a team player at all times. Must possess and maintain a South Carolina driver's license.
