About the Unit/Department/Team
The Acute Care Services Quality and Patient Safety Manager will work with a multi-disciplinary team across the system, reporting directly to the Chief of Nursing. This person should thrive working independently, but remain talented in connecting with other nursing leaders to strategize, implement and evaluate Patient Safety and quality initiatives.

The Acute CareServices Quality & Patient Safety Manager is responsible for the planning, development, implementation, coordination, and evaluation of Patient Safety and quality programs intended to increase Patient Safety, quality of care and patient satisfaction.
Position Summary
Strategic Priority: Develop System-Wide PS & QA Initiatives
Patient Safety
  • Responsible for system wide, organization, direction, coordination, implementation, education and evaluation of the activities associated with Patient Safety.
  • Identifies trends and conducts root cause analysis for the identification of process change.

  • Quality Improvement
  • Responsible for the coordination and organization of all quality improvement efforts in the Acute Care Division, and to make sure any QA initiatives are followed through by the division to achieve sustained high-quality performance

  • People Priority: Train, Collaborate & Communicate to Achieve PS & QA Goals
    Patient Safety
  • Works to ensure staff, specifically nursing directors and leads have a working knowledge and process for operational effectiveness with Patient Safety.
  • Works collaboratively with an interdisciplinary team to ensure the environment is centered on patients, families, safety and quality.

  • Quality Assurance
  • Works collaboratively with quality leaders across the system including the Senior Services Quality manager, the Performance Excellence & Talent division, and the Medical Director to develop methods and/or systems for improving quality care, service and measurement outcomes.
  • Communicates and collaborates with the leadership, physicians, extenders and staff as to the quality improvement plan and their respective responsibilities in carrying out quality improvement initiatives.

  • Operations Priority: Implementation, Evaluation and Reporting of PS & QA Initiatives
    Patient Safety
  • Maintains and is responsible for facility record keeping and reporting related to Patient Safety including occurrence reporting.
  • Identifies events that meet the definitions of an Adverse Health Event that require reporting through the Minnesota Hospital Association (MHA) Patient Safety Registry. Completes the root cause analysis for these events and enters the information into the registry.
  • Conduct, analyzes and evaluates the periodic Agency for Healthcare Research and Quality (AHRQ) Patient Safety survey system-wide and implements action plan based upon results.

  • Quality Improvement
  • Works collaboratively with Minnesota Department of Health, the Minnesota Hospital Association and other state and national Quality Improvement organizations to implement appropriate and required quality improvement reporting and activities.
  • Maintains and is responsible for facility record keeping and reporting related to quality initiatives.
  • Coordinates abstraction and submission of data from medical records per Center for Medicare and Medicaid Services (CMS) specification and other required or selected metrics from other Quality Improvement organizations including the Minnesota Hospital Association
  • Identify opportunities for improvement when measures are not met and implements actions accordingly.

    Qualifications
    Qualifications
    • Graduate from an accredited school of nursing.
    • Current RN license in the State of Minnesota
    • Current ACLS and PALS certification or obtained within 6 months of hire
    • Previous surgical experience required
    • Excellent communication skills.
    • Ability to plan, direct and/or implement and evaluate the nursing care delivered to patients.
