Responsible for providing care coordination services to engage individuals requiring behavioral and healthcare services. Care Coordinators will aid children and families toward ongoing treatment and recovery through education, outreach, and connecting clients to services. They will work in collaboration with the client, other SummitStone teams, and required health systems to provide support, link to resources, and fill treatment gaps as needed for youth and their families. Clients may be transitioning from emergency departments, other health services, primary care settings, emergency response services, acute care systems, crisis response teams and behavioral healthcare teams. Care Coordinators will work collaboratively with SHP staff, Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network, Community Health Centers, and a team of care providers to increase continuity of care for individuals seeking services across systems of care.
This Care Coordination position will work directly with the Fort Collins Community Health Clinic in assisting clients with MAT and behavioral health services.
Essential Duties:
- Effectively connect with and educate youth and their parents and guardians surrounding mental health and substance use needs and resources
- Coordinate and communicate care across providers and with the client & their guardians surrounding goals and resources
- Ability to work with youth who have been identified as struggling with truancy, mental health and substance use
- Support youth in individual and group settings
- Ability to work alongside and in collaboration with school district teams (counseling team, teachers, student resource groups, and community resources including but not limited to Matthews House, Center for Family Outreach, SummitStone Health Partners, the North Colorado Health Alliance, Juvenile Justice, human services systems and more).
- Work with youth in the community and provide wrap around care coordination services
- Ability to work with middle and high school youth.
- Ability to provide to students tier 1, tier 2, and tier 3 support
- Tier 1: Universal Interventions – available to all students in classroom or school
- Tier 2: Individualized Interventions – Subset of students receive interventions targeting specific needs.
- Tier 3: Intensive interventions - subset of students receiving intensive interventions.

- Required: At least 1 year relevant experience in the Mental Health Field, Substance Use Disorder Field, or program of application; experience with mild to severe mental illness and substance use disorders; experience navigating systems of care. Valid Colorado driver's license.
- Bachelors degree is required.
- Preferred: Bachelors (BA/BS) Degree in psychology, social work or related field from accredited College or University. CAS, CAT, CAC III or LAC.
Please note: The Care Coordinator will be required to obtain their CAT within their first 12 months of hire, or obtain their CAS within their first 18 months of hire.
