Job description

The Housing Stability area of the Human Services and Public Health department is seeking a Young Adult Housing and Connection Navigator to become part of the Homeless to Housing team.

The Homeless to Housing team will assist Young Adults, ages 18-24, experiencing housing instability. This position will maintain caseloads and work with Young Adults who are at-risk of housing instability, Young Adults staying in shelters or unsheltered locations and Young Adults fleeing domestic violence. Navigators will assist the Young Adults in being connected to resources critical to support their housing stability, as well as assist them in obtaining and maintaining permanent housing.

Effective December 31, 2023, there will be a general salary adjustment of 2.5% to the minimum and maximum of the range.

Location and hours:
This position is hybrid and will be performed both on-site at community sites within Hennepin County and remote as job duties require. Typical work hours are Monday through Friday, 9:30 a.m. - 5:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
  • Maintain a caseload of Young Adults and actively work to obtain permanent housing with them as a part of Homeless to Housing team services.
  • Provide services according to the 9 Evidence-based Guiding Principles to help youth overcome homelessness.
  • Work with Young Adults experiencing homelessness who may have mental health and/or substance use disorders.
  • Work closely with other Young Adult serving agencies to ensure collaborative and non-duplicative services.
  • Develop a youth driven Housing and goal plan.
  • Make necessary referrals and Connections to community agencies when appropriate.
  • If county resources are needed, collect information for eligibility review if needed to access these resources and assist Young Adults in navigating the system.
  • Help Young Adults in completing the State Medical Review Team (SMRT) process and in applying for Social Security Insurance.
  • Develop and cultivate relationships with landlords and property managers to connect and support Young Adults in obtaining and maintaining housing.

Need to have:
  • One of the following:
    • Two years or more of responsible, paid social service experience providing direct client service.
    • A two-year human services certificate.
    • Two years or more of college coursework in the behavioral sciences.
  • A valid driver's license and the ability to obtain a Hennepin County driver's permit, and access to reliable transportation for travel to various shelters and drop-in centers throughout Hennepin County and to transport clients to appointments and showings.
Nice to have:
  • Experience working:
    • Directly with individuals experiencing housing instability and/or homelessness.
    • With Homeless Management Information System (HMIS).
    • With the 9 Evidence-based Guiding Principles to help youth overcome homelessness.
  • Ability to:
    • Work effectively with external and internal partners.
    • Communicate effectively both written and verbally.
    • Work in a fast-paced environment requiring multitasking and strong time management skills.
  • Knowledge of community resources and social services available to clients.
