We are searching for a Wraparound/REACH Lead Care Coordinator!
Requirements:Applicants must have a vehicle and auto insurance, a bachelor’s degree in a relevant area of education or human services, at least one year of experience in the mental health field, at least one year of experience in a Care Coordinator role, basic computer knowledge, excellent writing skills and interpersonal communication skills, strong organizational and time management skills, and a welcoming demeanor and positive attitude.
Job duties include:
- Carries a maximum caseload of six families providing coordination of mental health services to help families meet their needs as stated in the Wraparound REACH care coordination job description.*
- Assists supervisor with providing consultation and supervision to care coordination staff including coaching staff by attending court hearings and Child & Family Team Meetings as required.*
- Supports supervisor in hiring, training, performance reviews, disciplinary actions, and dismissal of department staff.*
- Reviews and advises corrections on documentation for Care Coordinators on a weekly basis.*
- Ensures that all Wraparound Milwaukee and Agency-related policies and procedures are followed and implemented.*
- Assists Care Coordinators and supervisors with creating and implementing an agency improvement plan every six months while also reviewing progress towards reaching required thresholds on a monthly basis.*
- Assists the supervisor with tasks such as staff meetings, scheduling, organization, and delegation of responsibilities amongst staff.*
- Attends Wraparound Milwaukee supervisor meetings with the supervisor or as a backup when requested.*
- Provides oversight and direct supervision to the operational assistant including training on new tasks as needed.*
- Plans and coordinates monthly family events.*
- Assists the supervisor in maintaining the on-call procedure, policy, schedule, and training.*
- Supports Care Coordinators and families and teams during crisis situations as needed.*
- Responsible for assisting with APC agency functions such as MC3 goals and NIATx projects.*
- Responsible for faxing and filing documents.*
- Responsible for uploading PDF documents.*
- Other duties as assigned.*
