Department: Wound Care
Position Summary:
This is an on call, remote position that will be dispatched to appointments. The Wound Care Nurse Practitioner will be responsible for diagnosis and review of medical notes received from patients’ primary care physician, specialists, and those providing 30-day conservative treatment for the wound to determine a plan of action to provide Wound Care to patients. Care will be provided to patients in long-term, transitional care, assisted living and memory care homes. This position will be responsible for creating and implementing a plan, that results in successful Wound Care outcomes, while working closely with the facility nursing team. The Wound Care NP will also be responsible for doing clinical notes and must have them completed within 48 hours after patient visit.
Hours per Pay Period: Full-time with weekend availability, Monday-Sunday, On-Call 8am-6pm.
MUST HAVE A VALID DRIVER’S LICENSE AND MEANS OF TRANSPORTATION.
Pay: $500.00 per application
Education/Qualifications:
· Graduate of an approved Nurse Practitioner Program
· HIPPA complaint
· Certified by the American Academy of Nurse Practitioners or the American Nurses Credentialing Center or both.
· Licensed by the Illinois State Board of Nursing and in good standing.
· DEA License
· 5 + years of experience in Wound Care
· Wound, Ostomy, Continence Nurse (WOCN) certificate or ability to become certified within 3 months of employment.
· BLS and CPR certified
· Ability to react calmly and effectively in emergency situations.
· Ability to interpret clearly and establish/maintain effective working relationships with patients, medical staff, and facilities.
· Work well independently, in addition to being skilled in interdisciplinary collaboration.
· Is a self-starter, motivated, and demonstrates positive attitude.
Essential Functions:
· Ability to form and maintain relationships with facility staff.
· Exceptional written, verbal and interpersonal communication skills.
· Develop and manage an individualized Wound Care treatment plan for optimal healing outcomes or chronic maintenance.
· Collaborates closely with facility wound nurse and nursing staff on wound management practices.
· Knowledge of up-to-date topical Wound Care products and therapies, including ordering, application, and management of the products on wounds and ostomies, also including negative pressure wound therapy.
· Provides hands on Wound Care to patients including debridement of wounds by enzymatic, sharp, or autolytic methods when appropriate.
· Collaborate with the patient’s primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive plan.
· Update the primary care provider on the status and management plan of the wound as appropriate.
· Provides the required billing information for services provided by the Wound Care Nurse Practitioner and maintains the documentation necessary for those services billed.
· Reviews patient’s prior medical records and maintains their current medical record related to the wound diagnosis and contributing comorbidities.
· Patient chart encounters will be closed according to the standard of care.
· Assist with the identification of Wound Care quality measures and provide data and documentation for tracking of quality measures.
· Work with the Wound Care Nurse and facility leadership to determine opportunities for education of facility nurses with appropriate identification of wound types, prevention of wounds, and nursing care of wounds.
· Instruct patient and family regarding wound treatments and management plan for optimal wound outcomes.
· Triages telephone calls and provides consultation.
· Participates in professional development activities and maintains professional affiliations.
· Completes required number of continuing medical education (CME) hours for license and certification.
· Attends required meetings or committees as requested.
· Maintains patient confidentiality.
· Performs other duties as determined, collaboratively with team and administrators that are within the scope of practice of the Wound Care Nurse Practitioner.
· Maintains a copy of all professional records including current licensure, certification, continuing education, and other appropriate records.
Physical/Mental Demands and Work Environment:
· Involves exposure to communicable diseases, medical prescriptions, toxic substances, and other conditions.
· Requires full range of body motion to get in and out of car, as travel is required.
· Frequent standing, walking, and sitting required, for extended periods of time.
· Requires visual and hearing acuity.
· Able to lift a minimum of 25 pounds.
