Wound Ostomy Care Nurse - SBL Home Care
Job Description
Wound Ostomy Care Nurse is responsible for providing acute and rehabilitative care for patients with selected disorders of the gastrointestinal, genitourinary, and integumentary systems. Conducts research activities to improve patient care. Serves as a resource to patients and staff regarding Enterostomal Therapy.
Department: SBL Home Care
Hours: Monday-Friday, 8am-4:30pm, flexible scheduling with call every 3rd weekend
Requirements: RN, Driver's License, Wound Care Certified
Pay based on applicable experience; starting at $33.73
Responsibilities
Analyzes and recommends stock items for SBL Home Care to Materials Management services regarding equipment for ostomy, wound, skin and incontinence care., Assesses care and educational needs of individual ostomy patients. Educates patients undergoing ostomy surgery., Assessment- Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data., Conducts, utilizes and supports research activities and quality improvement studies related to care of patients for the purpose of improving clinical, fiscal and customer satisfaction outcomes., Coordination of Care- Plans and evaluates care in collaboration with appropriate disciplines for assigned patients., Diagnosis - Analyzes assessment data to determine actual or potential diagnosis, problems, and issues., Evaluation- Evaluates progress toward attainment of goals and outcomes. Reports data and outcomes to as appropriate to physician and/or other disciplines., Identifies the appropriate use of pouching and skin care products available for Home Care., Implementation - Implements care and supervises care delivered by other team members in alignment with the plan of care and approved safety and infection control standards., Improves the quality of patient care and nursing practice through a comprehensive approach to the management of patients., Knowledge - In collaboration with Leader/Director, actively pursues required knowledge and skills through orientation activities specific to the unit, unit based in-service programs, reading current literature and seeking new clinical learning opportunities., May have responsibility to perform duties in Home Health Services, Infection Control and Inpatient Services., Other duties as assigned to assist with overall operations of the agency., Outcomes Identification - Identifies expected outcomes for a plan individualized to the patient or situation., Participates in regular utilization reviews as required., Participates in staff orientation, competency assessment and ongoing education of staff regarding Ostomy Care, wound management, skin care, and incontinence management., Participates in the collection of data for Performance Improvement studies involving pressure ulcers, incontinence, ostomy diversion and wounds., Planning - Develops a plan that prescribes strategies to attain expected, measurable outcomes., Promotes the delivery of quality care to patients of SBLHome Care and persons in the community., Provides consoltation to staff members, as well as other hospitals, institutions and the community in the managment of patients with ostomies, draining wounds and fistulas, altered skin integrity and incontinence managment., Provides direct patient care as required., Provides educational programs to patients, families, and employees., Relationships - Establishes effective working relationships with peers, physicians, and other members of the health care team. Identifies and confers with appropriate resources regarding patient care decisions., Serves as a liaison with the medical staff regarding management of clients with ostomies, draining wounds, and skin care needs, and incontinence., Teaching - Employs teaching strategies to promote health and a safe environment.

ACLS - Advanced Cardiac Life Support W/I 90 Days - Sarah Bush Lincoln, BLS or CPR certification within 90 days of hire. - Sarah Bush Lincoln, Driver's License - Sarah Bush Lincoln, RN-Registered Nurse - Sarah Bush Lincoln, Wound Care Certified - Sarah Bush Lincoln
