Conexio Care CRISP—Wellness and Recovery Peer Navigator
Today, Conexio Care serves as one of Delaware’s largest nonprofits, collaborating with government, community, corporate, and other philanthropic partners to maximize services for our state’s most vulnerable The Program Ambassador is a member of the clinical team who assists clients and guests in gaining access to safe, timely, and quality services.
Conexio Care is looking for compassionate and eager professionals to join our team in a rewarding career working with adults with intellectual and developmental disabilities living in a residential group home. We are currently seeking qualified candidates to fill the role of Wellness and Recovery Peer Navigator. The Wellness and Recovery Peer Navigator is an individual who has a unique perspective because of his/her own experience with a serious and persistent mental illness, substance abuse, or a co-occurring disorder. S/he must be willing to self-disclose as appropriate and to attend training leading to certification.

Duties of the position:
- Promote recovery through modeling own personal self-confidence, achievements, and self-determination
- Provide mutual support, hope, reassurance, and advocacy
- Help clients aspire to roles that emphasize their strengths by sharing their personal experiences / stories
- Provide peer support to clients regarding their symptoms and the effects of trauma and trauma history, and helps them to develop positive coping skills and use of natural supports
- Engage clients through outreach and support
- Provide training and education to other team members on recovery-oriented care, peer support resources, psychiatric advance directives, wellness tools, trauma-informed care, and the use of creativity and the arts to promote recovery
- Attend interdisciplinary team meetings
- Assist person served to complete peer-related elements of the assessment and to prepare to attend their recovery plan meeting and to express their goals and needs
- Assist clients to accomplish their life goals of living in their chosen housing and community, working, having a natural support system, and having hobbies and other leisure activities which are creative and fun.
- Help clients and staff to develop and implement person-centered treatment plans.
- Facilitate peer recovery support groups.
- Act as an advocate for clients to obtain needed services and public benefits and to raise complaints and suggestions about unmet needs
- Provide on-call crisis intervention services as scheduled Attends the treatment team of all individuals for whom s/he is providing service and any individual that has been interested in receiving peer support
Required Education and Qualifications:
- High School Diploma or GED
- At least one year’s experience working in health services
- Experience working with underserved populations preferred
- Excellent communication skills and organizational skills
- Excellent ability to work with clients with substance abuse, co-occurring and/or mental health disorders
- Certification as Peer Recovery Specialist preferred but will consider a candidate with lived experience and a desire to pursue certification
- Must be available to work weekends.
- CPR and First Aide Certification
- Valid Drivers License in good standing with at least three years minimum driving experience with no serious traffic infractions
Additional Qualifications:
- Computer knowledge of MS Office, and other HR applications is a plus, not required.
- Possess strong verbal and written communication skills.
- Must be proactive, organized, and possess great follow-up skills.
- Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
