The purpose of the Weekend HC Admission RN is to provide and coordinate the delivery of home health care and services to patients/clients of the Agency through the application of the nursing process and in accordance with the philosophy, mission and standards established by the Agency.


Essential Functions:

  1. Develop, implement and evaluate outcome-oriented patient care plans requiring specialized nursing skills. Obtain family and/or patient involvement in the development of such plan whenever possible; complete all necessary documentation, according to Agency policies and procedures.
  2. Conducts initial evaluation visits and regularly re-evaluates the nursing needs of the clients; initiates, develops, implements, and make necessary changes to the client’s plan of care. Addresses all problems in the plan of care or documents rationale for not doing so.
  3. Initiates diagnostic, preventive, and rehabilitative nursing procedures as appropriate to the client’s care and safety. Makes referrals to other disciplines as indicated by the client’s needs or documents rationale for not doing so.
  4. Assesses client’s continual care needs and informs physician and any other appropriate health professionals, as often as needed, of any changes in the client’s conditions.
  5. Teaches supervises, and counsels the client and family regarding home nursing procedures and other care needs as appropriate to the client’s condition. Utilizes Agency educational material, as appropriate. Ensures infection control and safety measures are identified, implemented, and documented.
  6. Provides supervision of the plan of care for home health aide services according to Agency policy when necessary. Assess the client’s need for unskilled care and revises plan of care, as needed; reports any changes to the plan of care to the Case Manager or the Clinical Manager as often as necessary.
  7. Maintains accurate, timely and legible records and reports.

Non-Essential Functions:

  1. Participates in continuing education and in-service activities designed to improve professional services and personal, professional growth.
  2. Participates in Agency-conducted community education activities by supporting and/or providing input and appropriate expertise for various programs.
  3. Baylor staff will have regular schedules covering weekend hours. They will work two twelve hour shifts per their agreed upon schedule with the Weekend Clinical Manager. They will rotate through 5 pm to 8 pm on call as assigned by the Clinical Manager. They will have a Holiday committeemen per agency policy. The twelve-hour shift productivity is 9 points if the RN is taking on call the productivity is 7 points. If working an eight hour shift, the productivity is 6 points.
  4. Assures Agency compliance with all federal and state laws as well as adhering to the Norwell Visiting Nurse Association, Inc. Corporate Compliance policy in the prevention of fraud and abuse on behalf of the Agency.
  5. Performs other Agency-related duties, as assigned.

Must be able to appreciate the needs of the Agency’s patients despite where they live must be met and travel of the RN may be necessary.

C. Other

  • Flexibility in work schedule and/or assignment to meet time requirements of the position (which will include evening/weekend/holiday).
  • Completes appropriate competency and/or certification/re-certification testing.
  • Ability to work independently with minimum supervision.


Education:

  • Current Massachusetts License (in good standing)
  • Registered Nurse (Bachelor’s Degree preferred)
  • Current CPR Certification, highly recommended

Experience

  • 2 plus years of experience in the acute care setting preferred
  • Previous community health experience preferred
