Required Education and Experience:Recent Graduates are Welcome!!

Valid Registered Nurse's license from the Commonwealth of Massachusetts,

Valid CPR certificate,

Valid driver's license

Essential Functions:

*Conducts home visits as needed including clinical assessments, initial assessment and re-assessments.

*Develops Care Plans that are appropriate to meet the member's needs
*Complete and prepare all necessary assessments, data, forms and care plans as required
*Keeps updated progress notes in accordance with EOEA documentation standards.
*Works closely with Director, Coordinators, Care Managers, Caregivers, Members to coordinate services.
