Job Title/Position: Psychiatric Nurse (RN)/Case Manager

Pay: $55 - 60 per regular visit (dependent upon experience), $65 - $70 per behavrioral health visit, $100 per start of care

Benefits: Mileage and travel reimbursement. Company paid cellular data. Health, Dental, Vision & 401k (with match) benefits available.

BRIEF JOB DESCRIPTION:

The Psychiatric Nurse (RN) provides nursing care to the patients/clients as requested by the patient/client and, when required, as prescribed by the physician and in compliance with applicable laws and regulations and the policies of the Organization.

The Psychiatric Nurse (RN) is responsible for traveling to a patient’s home to administer the services dictated by their physician while helping patients maintain their independence and quality of life. This role provides a lot of FLEXIBILITY for our field staff!

RESPONSIBILITIES AND DUTIES

1. Seeks and obtains sufficient information necessary to function effectively in the health care setting where assigned.

2. Provides professional nursing care by utilizing all elements of nursing process.

3. Assesses and evaluates patient's/client's status by:

A. Establishing and implementing the plan of care

B. Regularly re-evaluating patient/client needs

C. Reviewing and revising the plan of care as necessary, at least every 60 days.

4. Administers medications and treatments as prescribed by the physician.

5. Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques and maintains communication regarding patient/client care with the physi¬cian and supervisory personnel.

6. Completes, maintains and submits accurate, relevant clini¬cal notes using patient-specific

pathways regarding patient's/client's condition and care given, within 24 hours of the visit.

7. Uses supplies and equipment effectively and efficiently.

8. Observes the patient/client for changes in condition, discusses changes with the patient/client or family/care giver and reports changes, as necessary, to the attending physician and the nursing supervisor.

9. Evaluates the patient’s/client’s activities of daily living and facilitates the patient's/client's efforts toward self-sufficiency and independence.

10. Teaches the patient/client and family/care giver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/clients and families/care givers.

11. Provides and maintains a safe environment for the patient/client.

12. Works with the patient's/client's family/care giver and other health care personnel in providing continuity of care. Coordinates multi disciplinary case conferences at least every 7-14 days.

13. Makes intermittent home nursing visits to assigned patients and provides skilled nursing services

as ordered per the physician.

14. Specifies in writing, the assignment for Home Health Aides. Supervises the HHA every 14 days. Reviews and revises assignment plan as necessary to meet the patients needs at least every 60 days.

15. Supervises LP(V)Ns at least every 30 days or as required by the patients needs.

16. Supervises ancillary personnel and delegates responsibilities when required.

17. Takes on-call as assigned.

18. Performs within the limits of preparation and experience.

19. Accepts responsibility of assignment to perform a specialized procedure, such as IV therapy, only when qualified with specialty training, proven competency, clinical back-ground, and expertise in the specialty area.

20. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continu¬ing education and inservice classes.

21. Fulfills the obligation of requested and/or accepted case assignments.

22. Functions as the case manager of the case and coordinates all disciplines, care and treatments. Writes a 60-Day Summary of Care addressing all disciplines in the home at least every 60 days or as necessary due to the changes in the patient’s condition.

23. Responsible for chart updates. Assures there are physician orders for all disciplines/

therapies/treatments.

24. Communicates with insurance case managers regarding patient status and precertifies visits. Documents this communication in the patient’s chart.

WORK ENVIRONMENT

1. Works in community home environment.

2. Exposure to infectious diseases and biohazardous materials

3. Able to drive for long periods (one to two hours) of time.

4. Goes in and out of automobile in cold and inclement weather.

5. May occasionally have to lift/carry up to 50 pounds.

6. Ability to transfer patients weighing up to 200 pounds.

7. Ability to walk one to two blocks, sometimes on uneven ground.

8. Ability to stand for periods up to one hour.

9. Must be able to squat, kneel, and bend.

10. Must be able to use hands for grasping, turning, and fine manipulation.

POSITIONQUALIFICATIONS:

1. Is a graduate of an accredited school of professional nursing. BSN preferred.

2. Is currently licensed as an RN through the State Board of Nursing.

3. Is eligible for Medicare approval as psychiatric nurse:

A Registered Nurse with a Masters degree in psychiatric or mental health nursing, must have recent nursing experience in an acute treatment unit in a psychiatric or mental health hospital or outpatient program (including psychiatric home care).

OR

A Registered Nurse with a bachelor’s degree in nursing with one year of recent nursing experience in a active treatment unit in a psychiatric or mental health hospital or outpatient program (including psychiatric home care).

OR

A Registered Nurse with a diploma or associate degree with two years nursing experience in an active treatment unit in a psychiatric or mental health hospital or outpatient program (including psychiatric home care).

OR

Successful Medicare certification on an individual basis (other combinations of education and experience may be considered by Medicare Fiscal intermediary).

4. Has at least one year's general nursing experience, with current home health, medical, surgical, or critical care experience.

5. Complies with accepted professional standards and principles. Follows all rules/regulations in home care as mandated by state/federal regulations.

6. Possesses and maintains good physical stamina and mental health. Has presented a pre-employment physician's health clearance including a negative TB skin test and/or CXR and other tests as required by the Organization's policy.

7. Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements or has a reliable mode of transportation.

8. Possesses and maintains current CPR certification.

9. Has satisfactory references from nursing school, previous (or current) employers and/or

professional peers.
