Job description

Position Summary:
The VP of Senior Health Services is responsible for overseeing comprehensive delivery of geriatric and eldercare services for the PACE Program. The VP of Senior Health Services reports to the President and CEO and serves as SYHC's principal advisor on leading and directing SYHealth geriatric and eldercare medical and social services. The VP of Senior Health Services in concert with the President and CEO is responsible for the daily planning, implementation, operation and expansion of the PACE Program. This includes ensuring that appropriate personnel perform their administrative, clinical, operational, professional, financial and programmatic functions within the PACE setting to support the respect, dignity and independence of SYHealth's frail elderly PACE participants. This position is responsible for managing all aspects of the PACE Program, which consists of a primary care medical clinic, provider network, 24 hour care delivery, preventive services, therapeutic services, nursing services, day center services health Program, home care, social worker services, transportation, nutrition, and QA activities.
Essential Functions:
  • Working in concert and conjunction with the President and CEO oversees and manages the following clinical, operational, financial and support components and responsibilities of the PACE Program.
  • Oversees the PACE Medical Director, PACE Director, Marketing Director, and Quality and Compliance Manager.
  • Oversees and administrates all facets of the PACE Program, including effective planning and implementation of all program operations.
  • Develops and manages all external program relations. Represents the PACE Program in the community and on the state and national level and assures positive public relations.
  • Develops a PACE delivery model specific to the needs of the San Diego PACE service area.
  • Secures all necessary PACE funding, including start-up funding, Medicare and Medicaid reimbursement, and overall program fundraising.
  • Obtains and maintains PACE certification under CMS, as well as any other state or federal laws or rules. Assures compliance with all state and federal regulations and works with appropriate governmental agencies regarding regulatory requirements or changes.
  • Initiates and implements PACE strategic planning processes and resources both internally and within the local community, including planning for program start up, expansion, and creation of new programs or services.
  • Oversees and ensures overall PACE program quality and effectiveness, including identifying the need for services and innovative ways to provide care, improving efficiency and effectiveness, and responding to participant and community needs.
  • Develops and implements written policies and procedures that govern the operation of San Diego PACE and ensures compliance with PACE provider regulations.
  • Oversees, directs and supervises San Diego PACE key personnel, both salaried and contractual.
  • Reviews and approves the appointment, transfer, or termination of all key staff as defined by DHCS. Notifies the State of California in a timely manner when these changes occur.
  • Provides leadership and direction to the PACE Management.
  • Ensures the financial health of the program, including assisting in preparing an annual operating budget for approval by the Board of Directors, allocating the resources to carry out operational and programmatic activities, reviewing and interpreting monthly financial statements, developing actions to correct areas of concern and developing strategies for further growth and development of San Diego PACE.
  • Endeavors to sponsor and promote the design, organization and operation of the PACE primary care clinic as Patient Centered Medical Home (PCMH) Models of Care and advanced primary care practice settings.
  • Champions developments, improvements and operation of the Electronic Health Record (EMR) system for the PACE Primary Care Clinic (Patient Centered Medical Home), PACE eldercare continuum of care (PACE Site, provider network and senior patient home and long-term care settings).
  • Establishes and maintains an effective system of communication and emphasizes teamwork throughout the San Diego PACE organization.
  • Represents San Diego PACE locally, statewide and nationally including attendance and active participation in Cal PACE and National PACE Association.
  • Identifies annual census goals and ensures the enrollment structure/process is streamlined and conducive to reaching/exceeding those goals.
  • Collaborates with SYH clinics to ensure coordinated medical care.
  • Works with other community based organizations to address non-medical PACE participant needs.

Additional Duties and Responsibilities:
  • Performs other PACE duties as assigned by the President and CEO.

Job Requirements:
Education Required (Minimum level of education): Master's degree preferred in a related field, including, but not limited to, health care administration, business administration, social services, or nursing - or equivalent knowledge, skills, and abilities experience.
Training and Experience Required (Minimum level of experience):
  • 3 - 5 years of work experience in health care and/or long term managed care, preferably for an at-risk population and in a capitated setting.
  • Proven experience necessary in program development, strategic planning, budgetary oversight, organizational development, resource development, and public relations.
  • Strong leadership, management and organizational skills, including skills in community collaborations are essential.
  • Strong familiarity with multi-disciplinary programs serving culturally diverse populations is preferred.
  • Bilingual/Bicultural in Spanish/English preferred.

Skills and Knowledge Required to Perform the Job:
  • Strong personnel and health care financial management skills.
  • Skills to optimize relationships with other organizations and programs.
  • A comprehensive knowledge of organizational policies, procedures, and systems.
  • Strong analytical and negotiating skills.
  • Effective verbal and written communication skills and good organizational skills.
  • Possess a current CPR certification by the American Heart Association or equivalent or willingness to obtain as offered by San Diego PACE.
  • Immunizations and vaccines must be current or medical documentation from a medical provider to request a waiver.
