FUNCTION
This position is responsible for supporting program participants. They would be responsible for ensuring clients receive services in relation to work readiness and successful transition.
TOP RESPONSIBILITIES
● Providing individual case management sessions with all clients at regularly scheduled intervals; and providing referrals and any other needed services.
● Coordinate with AFC housing and drop-in case managers to assist clients in obtaining identification, mental health care, medical care, and stable housing and make appropriate referrals
● Participate in client recruitment; Provide intakes and assessments of UW clients
● Developing Individual Service Strategies with each client to address life skills, employment, educational and vocational needs.
● Track weekly participation of up-to-UW clients and maintain the PTS database
● Create and support facilitation of a client-led peer support group.
● Create and facilitate workshops on topics that are relevant to the needs of clients
● Responsible for daily record keeping and documentation, maintaining client charts & compiling statistics for monthly program reporting; data entry.
● Coordinate with administrative support to identify disengaged UW client
● Attend weekly all staff meetings and individual supervision.
● Assist AFC Housed clients with weekly residential visits
● Assist in the flow of services at the Drop-In Center & other duties, as assigned.
EDUCATION REQUIREMENTS

[ ] Vocational Training
[ X ] Undergraduate Degree
[ ] Masters Degree
Training/Experience in youth social services required.
TECHNICAL SKILL REQUIREMENTS
Computer knowledge and competent writing skills.
PREFERRED QUALITIES:
A bachelor's degree in social work, counseling, education, or related relevant field and a minimum of three years of experience within the last five years providing case management, and workforce preparations services to RHY & LGBTQI+ youth. At a minimum, the navigator would also have at least three years of trauma-informed counseling; experience managing caseloads of 20 or more clients; and successful experience creating and maintaining strong working relationships with other organizations for referral, case conferencing, and shared resources.
