Responsibilities:
The Triage Registered Nurse (Triage RN) is a qualified professional and is an employee of the Agency. The Triage RN is responsible for providing telephonic triage to hospice patients and their families during after business hours, weekends, and/or holidays. The Triage RN is responsible for coordination and professional nursing functions to assess the severity of the patient’s health concerns in order to dispatch appropriate licensed staff members.

Essential Duties and Responsibilities
  • As scheduled, will work to perform nursing assessment of each patient across Hospice division
  • Prioritize treatments according to urgency of need and initiate medical care
  • Coordinate with local vendors and site team to ensure timely delivery of pharmacy, DME and medical supplies in order to meet patient care needs
  • Ensure after-hour visits are scheduled with the on-call team in a timely manner
  • Provide medical care to include medication orders for patients for comfort while dispatching field staff as needed
  • Coordinates, assigns and dispatches local nursing staff to complete new admissions and routine visits as needed
  • Process new after hours and weekend patient referrals when applicable
  • Assist local office site with outstanding workflow in HomeCare Home Base as needed
  • Collaborate with site Hospice Medical Director to discuss admissions and patient care decisions
  • Provides home care instructions using the approved, written guidelines as well as approved reference material provided
  • Utilize all resources and guidelines to effectively assess, prioritize, advise as necessary
  • Respond promptly to each incoming call and dispatch appropriate staff member per guidelines
  • Document symptoms, complaints, nursing assessment, advice provided and patient/caller response in system immediately
  • Report On-Call activities to the clinical leadership at the end of the call shift as required or needed
  • Attends staff meetings and educational in-services per agency requirements
Qualifications:
Required Knowledge, Skills and Experience
  • Current unencumbered Multi-State Registered Nurse (RN) state license or in accordance with the
    Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
  • Able and willing to obtain Registered Nurse (RN) state license or in accordance with the Board of
    Nurse Examiners rules for Nurse Licensure Compact (NLC) in all applicable HarmonyCares
    Hospice states within 6 months of date of hire
  • 1 or more years of health care triage to include hospice, ICU or critical care unit experience
  • 1 year of Electronic Medical Record experience
  • Excellent written and verbal communication skills
  • Ability to work in a support setting and exhibited ability to make sound nursing judgments to
    include but not limited to clinical assessments and orders
  • Ability to asses patient needs and formulate individualized patient care plans to meet those
    needs telephonically
  • Working knowledge of computer programs (email, Word, Excel, PowerPoint, etc.)
  • Must be self-motivated, independent, structured, organized, very detailed and able to meet
    deadlines
  • Ability to effectively communicate and collaborate with others
  • Ability to multi-task in a demanding and constantly changing environment
  • Demonstrate flexibility in addressing changing patient needs and program environment
  • Ability to gain respect and build rapport with patients and their families along with staff members

Preferred Knowledge, Skills and Experience
  • Bachelor of Science in Nursing
  • Hospice experience
  • Experience in HomeCare HomeBase
