Provides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items.
Pay Rate: $2,000.00 - $2,200.00 gross per week

Start Date: Ideal candidate can start as early as 04/15/2024. Will take candidates who can start before or after
Assignment Length: 13 Weeks
Schedule: Days / 5x8 Hour Shifts / 40 Hour Guarantee / Rotating Weekends as needed (2-3 per month)

REQUIREMENTS
- 2+ Years Nursing Experience - REQUIRED
- Minimum 6 Months of Home Health Experience - REQUIRED
- Travel Experience - REQUIRED
- Active and Clear Driver's License and Car Insurance - REQUIRED
- Homecare Home Base and OASIS - REQUIRED
- COVID and FLU vaccine - REQUIRED
- Bachelor's degree - Preferred
- Case Manager experience - Preferred
- Mileage reimbursed at .43 cents per mile
CERTIFICATION REQUIREMENTS
- BLS (AHA)
STATE LICENSE REQUIREMENTS
- Hawaii
