About Us:

Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network works together to create opportunities for Ohioans to receive needed home and community services and support and to age successfully in their homes and communities.

Position Summary:

The Transitional Care Home Coach may be a RN, LSW, or LPN and is an integral part of the member centric TC Team as the initial introduction between members and Western Reserve Area Agency on Aging. Their role is essential to support the promotion of the members' goals of well-being and independence and the managed care organizations' collaboration and coordination of services. The successful Home Coach will act as part of the TC Team to provide member-centered coordination of services.

 

Job Duties: 

  • Introduce and explain the specific program elements to program participants that includes medications, scope of care, scope of transition event, coaching process, program expectations and program /agency resources.
  • Completes program participants visit in the home or hospital, in person, videoconference, or telephone as required by program.
  • Conducts and documents all prescreening assessments, chart history, and initial intake evaluations for each program participant.
  • Provides coaching for the participant related to personal goals, conditions, medication compliance, medical appointment follow-through, and self-advocacy.
  • Provides education and support to program participants and their caregivers about Agency resources.
  • Advocates on behalf of participant and/or caregiver/family and assists in securing appropriate services as outlined in the participant's insurance policy and available agency resources.
  • Applies clinical knowledge and critical thinking to perform program activities based upon education, experience, and respective professional practice standards.
  • Reports incidents of abuse, neglect, or exploitation of older adults as a mandated reporter as required by Ohio law and program guidelines.
  • Abides by the professional code of ethics established by the Ohio Board of Nursing, Ohio Board of Counselor, Social Work, Marriage & Family and WRAAA.
  • Consistently demonstrates effective customer service behaviors and practices with internal and external stakeholders to meet department goals, program metrics and agency mission.
  • Participates in training, meetings, in-services, and continuing education opportunities as offered by the agency, managed care partners, or as identified by the employee. Also as requested by supervisor, manager, or director to meet departmental goals.
  • Ability and willingness to travel to community and participant locations within all county service areas (Cuyahoga, Lorain, Geauga, Lake, Medina, and Summit counties).
  • Collaborate with external agencies, internal departments, and partner AAAs to ensure continuity of care with program participants.
  • Perform other duties as assigned by TC Management as needed to meet agency and departmental goals.
  • This position's primary duty is to perform work related to the successful operation of the Agency. Accordingly, the employee must exercise discretion and independent judgment with respect to matters of significance.
  • All staff have the responsibility and authority to propose ways to improve quality and to fully participate in quality improvement efforts.

 

 Requirements:  

  • Current, unencumbered, and unrestricted licensure as a Registered Nurse, Licensed Social Worker, or Licensed Practical Nurse in Ohio. 
  • At least one year of clinical experience post licensure.
  • Is aware of the agency's goals and works to support the agency's mission and vision.
  • Must demonstrate professional behaviors consistent with WRAAA culture.
  • Must have excellent customer service skills in person, in writing and on the phone while conducting interviews, assessments, and practical skills.
  • Must have excellent interpersonal and communication skills.
  • Must complete all training, assigned, or required, to maintain credentials to all TC software programs.
  • Intermediate knowledge of standard precautions and use of personal protective equipment, major chronic illnesses for persons of all ages, disease processes, and normal aging or special knowledge in pediatrics and young adults.
  • Ability to effectively communicate with all populations in person, by videoconferencing and telephonically.
  • Ability to communicate collaboratively with others internally and externally and to maintain good working relationships.
  • Ability to comprehend written material related to essential job functions.
  • Ability to comprehend basic medical terminology.     
  • Ability to analyze, evaluate, and implement a reasonable course of action based on available information.
  • Ability to detect specific odors related to the assessment of a client's health/hygiene and the safety of the client's environment.
  • Must have basic computer skills, including Microsoft Office applications and is expected to participate in training and use tools to improve skills to an intermediate level.
  • Ability to use and transport PPE, a laptop computer and case, therefore must be able to lift/carry a minimum of thirty (30) pounds.
  • Must have a car available during working hours, as well as the ability to legally drive a car and have current insurance on the car used for agency activities.
  • May sit, stand, stoop, bend, and ambulate intermittently during the day.
