Job Description
A qualified Transition Youth Specialist will provide a range of independent living and vocational development services to students in 8th – 12th grade experiencing behavioral, emotional and/or social challenges, aiding their transition from school to work and independence. These school and community-based activities and interventions include initial and ongoing evaluations of students' strengths, limitations, desires, and interests; collaborating with other service providers and school staff to address student needs; engaging students enrolled but not actively participating in school; assisting students in obtaining internship/community service hours for graduation; assessing students' Independent Living Skills; identifying and cultivating relationships with community employers for vocational development activities; connecting students and families with community resources and facilitate access to services.
Staff is required to transport participants in their personal vehicle as needed.
