Summary:
The Transitions of Care (TOC) Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. The pharmacist will perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge. Preferred Experience is PGY-1 residency; PGY-2 residency.
