Adaptive Home Health is looking for a Therapy Care Coordinator to join our fast-growing team providing high quality care to patients throughout Texas. Within this role you will have an opportunity to both strengthen your knowledge of the home health care industry and grow alongside a team of dedicated professionals.

Responsibilities:
Work with the clinical team in the creation of a patients therapy schedule
Timely communication with vendors throughout the day
Coordinate with and assign therapists
Field patient calls to address any therapy scheduling concerns
Work with clinical team to ensure patients are seen in accordance with their plan of care
Communicate schedule changes between office staff and field staff

Requirements:
Successful background check
Drug Screen
Proficiency with Microsoft Office
Customer service experience
1-2 years experience (preferred)

Job Type: Full-time

Pay: $16.00 - $19.00 per hour
