Therapist/ Clinical Case Manager

This position is temporary to start but can lead to a direct hire position. Will be temp for minimum of 4 months.  Agency is growing and there is excellent growth potential within agency. Clinical supervision is offered. Hours are basic business hours but can have some slight flexibility, clients are seen mainly between 10AM - 3PM, M - F. This is full time hours, day only, no evenings, weekends or on call.

As a Therapist/ Clinical Case Manager you will be at the forefront of transforming lives and making a difference in the field of mental health. The Therapist/ CCM is responsible for delivering exemplary patient care by conducting comprehensive psychosocial assessments, formulating interdisciplinary treatment plans, leading 2-3 group therapy sessions per day, and overseeing the discharge and aftercare planning process. Participate in weekly treatment planning for each patient, works cooperatively as a member of the interdisciplinary treatment team, completes progress notes and discharge plans. Provide invaluable support to patients and their loved ones by employing advanced psychotherapeutic skills to address and resolve intricate challenges arising from psychiatric conditions.

Qualifications:

ASW/ AMFT

• 1+ year of experience working with individuals with a history of mental illness in a social service/mental health environment.
• Demonstrated knowledge of Evidence Based Practices and/or the Recovery Model of treatment.
• Proficiency in group therapy facilitation to create a supportive and therapeutic environment for clients.
