Sentara Health Plans is hiring a Temporary Integrated Nurse Case Manager in Richmond Virginia!

This position has full time hours and will last for 6 months with the potential of extending and going permanent. The position requires both in person face-to-face assessments and remote telephonic assessments!

Position requires in person assessments to be done in members homes

Applicants must be located in the Richmond, VA or surrounding area

Area includes: Richmond, Glen Allen, Ashland, Fredericksburg, Midlothian

As an Integrated Nurse Case Manager you will perform a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including at time of terminal illnesses.
Minimum Qualifications:

• RN License (Virginia)
• Associates or Bachelors Degree in Nursing
• 3 years experience in Nursing

Preferred Qualifications:

• Case Management experience
• Experience working with Waiver Population
