Theoria Medical is seeking dedicated physician assistants and Nurse Practitioners to join its telemedicine team to provide high quality care at the skilled nursing facilities (SNF) it manages. We have a wide variety of roles available suited for providers in various stages of their careers with flexible scheduling in a positive work environment.
Shift Structure
Available shift types are as follows:
- Night shifts 7pm-7am available daily
- Day shifts 1pm-7pm available Monday-Friday
- Day shifts 7am-7pm available on Weekends only
- Full time staff are required to work a minimum of facilities per shift
- Telemedicine shifts are not a traditional "on-call." Staff must be awake and alert at all times
- This is a remote position
Compensation and Benefits
- Full-Time
- $200-300/6hr and $500-600/12hr Shift rate based on number of facilities covered

Essential Functions and Responsibilities
- You will provide telemedicine on-call coverage to skilled nursing facilities
- You must maintain a relationship with facility nursing staff/patients that is reassuring and professional while answering questions and performing examinations
- Perform telemedicine video consults when appropriate
- Continuously monitor and answer incoming secure messages from facility nursing staff
- Medication verifications and review for new admissions to the facility
- Determining the appropriate course of medical treatment for patient by evaluating symptoms, prescribing medications, order labs, order imaging and ensure proper follow up care is arranged
- Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™
- PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems
Requirements and Qualifications
- Candidate must hold or have applied for PA/NP licenses in the following states:
- Michigan
- Ohio
- Indiana
- North Carolina
- Kentucky
- New Hampshire
- Texas
- Tennessee
- Wisconsin
- Candidate must hold a valid NP/PA degree from an accredited institution
- Candidate must have an active DEA license (If new graduate, Theoria will assist with your application)
- General Post-Acute/SNF knowledge or experience preferred
- Candidate must be familiar with EMR systems and charting practices
Physical Requirements
- Must be punctual or on time and adhere to the company's Time and Attendance policy.
- Must be able to remain sitting for the majority of their shift.
