How You Will Make a Difference: Collaborating with our dynamic team, you will assist in the development and implementation of personalized Care Management Plans for GrayHawk Health's diverse patient base. Your responsibilities will extend beyond routine Occupational Therapy assistance to encompass educating patients on condition management, providing behavioral support, and addressing the emotional challenges associated with chronic conditions. By working crlosely with patients to set and achieve goals, your efforts will contribute to enhanced self-management outcomes.

What You Can Expect from Us:
Salary Range: $20.00 an hour to $25.00, based on experience.
Comprehensive Health/Vision/Dental Insurance effective from the 1st day of the month after your start date.

What We Require of You:
Age Requirement: At least eighteen (18) years of age.
Work Authorization: Authorized to work within the United States.
Experience: 1+ year of experience in Case Management is preferred but not mandatory. Your passion for making a difference matters most.
