Job description

Position summary:
This is a grant funded position. Diabetes Program Nurse Practitioner is primarily responsible for providing medical management, care and education for persons diagnosed with diabetes and related chronic conditions, as well as prevention and wellness. This position will also assist with ongoing program planning, development and implementation of diabetes-related chronic disease prevention and treatment projects Conduct regular trips to village clinics to provide patient care. Collaborate with YKDRH and Sub-regional clinic providers to ensure patients are receiving care according to American Diabetes Association's Standards of Care. Provide technical assistance and training to medical staff on Diabetes issues and current standards of care. Assist with diabetes-related health promotion projects, visit tribal councils, and function as a Diabetes Team Member. Believe in and promote Napartet and all corporate endeavors.
Position Qualifications:
  • Master's Degree in the Science of Nursing; preferred Master's Degree in Public Health (MPH).
  • Two (2) years experience in family practice, chronic disease management or related clinical setting. Preferred experience working in public health programming. Experience providing patient care and/or teaching in a cross-cultural setting. Experience working with database and case management. Experience collaborating with a variety of disciplines for project completion.
  • Nurse Practitioner, national certification, licensed and in good standing or eligible for licensure in the State of Alaska. Nurse Practitioner must be eligible for a DEA number and eligible for credentialing at YK Delta Regional Hospital in Bethel.
    Preferred Certified Diabetes Educator (CDE) or Board Certified in Advanced Diabetes Management (BC-ADM).
  • Work in a typical office, hospital and field clinic settings. Travel to villages via small plane/sub-arctic conditions, moderate lifting of equipment, occasional long working hours and overnight stays in village clinics up to a few days are required.
  • Strong knowledge base in the field of diabetes, cardiovascular disease and public health. Computer skills and organizational skills. Preferred bi-lingual (Yup'ik/ English). Ability to work in a group setting, ability to work in remote villages. Ability to work with people of different cultures/backgrounds. Ability to be flexible and handle stress
