Description
The Supervisory Case Manager - Rapid Rehousing Plus Care Housing Program will oversee advocacy, crisis intervention, support services, and coordination with funding sources for the following programs: Raping Rehousing Plus Care. Ensures that their assigned programs meet or exceeds program objectives, oversees the provision of high-quality services for program participants, and further ensures on-going compliance with funder requirements and agency policy and procedure. Responsible for the recruitment, training, supervision and scheduling of staff, volunteers and interns.
Supervisory Case Manager - Rapid Rehousing Plus Care Housing Program will also be responsible for conducting outreach in conjunction with the team, and all aspects of case management, including accessing available benefits, obtaining and coordinating other public benefits, managing temporary financial assistance and providing other supportive services for participating households. In conjunction with the team, develops and maintains a comprehensive list of applicable resources and other services for referral purposes. Acts as a knowledgeable liaison to community agencies and is responsible for promoting awareness of the Volunteers of America resources and services. Position requires ongoing interaction with team members, program participants, and community stakeholders. May also be responsible for a small caseload of participants for case management purposes.
Essential Duties and Responsibilities
- Ensures that the participant's voice is heard and incorporated in programming with a trauma-informed lens.
- Responsible for the on-going monitoring of service provision, including oversight of all operational program components from outreach to program exit.
- Recruits, hires, trains, coaches and supervises assigned program and support staff and implements on-going training and staff development activities.
- Responsible for continuous program development and oversight, reviewing funder requirements on an on-going basis.
- Oversees participant program eligibility certification and requests for temporary financial assistance, ensuring all funder and agency requirements are met prior to payment.
- Reviews and monitors budgets monthly to ensure proper allocation of funds for all programs within this position's purview.
- Works closely with managers, supervisors and others to ensure team members are receiving appropriate support and have a comprehensive array of referral sources.
- Assists team in coordinating and connecting with services for program participants.
- Supervises the provision of support services provided by team members, ensuring adherence to program requirements and field-relevant best practices and standards.
- Monitors and performs scheduling functions ensuring adequate coverage for participant accessibility, and schedules staff for outreach activities in conjunction with team members.
- Monitors all aspects of program documentation completed by assigned staff. Gathers statistical data relevant to the program as required.
- Acts as a community liaison maintaining cooperative working relations throughout the community.
- Oversees all intakes, housing orientations, lease-ups and ongoing educational opportunities for participants in program. Assists in relocating households as needed and securing emergency housing.
- Develops specialized knowledge related to service delivery database systems within the organization. Ensures timely reporting within data entry systems and hard copy files in accordance with grant guidelines.
- Assists with community-wide outreach to identify eligible participant households presenting with substantial barriers to housing stability. Works with community coordinated entry system to request and receive program referrals.
- Facilitates the delivery of appropriate services for all participant households as indicated by relevant service modalities through an equity lens.
- Demonstrates knowledge of resources for unhoused persons, a passion for serving people experiencing homelessness, and the ability to contribute to the program-wide objectives of a diverse and dynamic team.
- Performs comprehensive case management assessments and develops plans of intervention that addresses housing retention, services, service gaps, crisis intervention, increase in access to income/public benefits and continuity of care planning and coordination, including coordination with clinical services.
- Point of contact in crisis intervention and de-escalation as client needs arise.
- Develops, directs and monitors the implementation of quality Housing First/CTI-based supportive services tied to housing stability. Coordinates with the entire team to deliver services most appropriate to meet the needs of presenting households.
- Attends orientations, trainings, education programs, staff meetings, community meetings, conferences and workshops as requested and applicable to meet the needs of the position. May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices.
- Embodies meeting participants where they are, literally and figuratively. This may include meeting participants in the community and transporting participants for case management purposes.
- Performs duties in a professional manner by maintaining the confidentiality of all information and by participating effectively within and across teams.
- Performs job responsibilities in accordance with the Social Work Code of Ethics.
- Performs all other duties as assigned.
Position Type and Expected Hours of Work
Full time 40 hours per week Monday through Friday. Some evenings and weekends required.

Minimum Qualifications of Position
- Bachelor's degree in human services, social work, or a closely related field or related experience.
- At minimum, two years of working with marginalized communities.
- Demonstrated experience in leading teams.
- Colorado driver's license and state-mandated automobile insurance.
Preferred Qualifications of Position
- Previous management experience.
- Familiarity with current housing market information, housing trends and available resources.
- Experience working with people experiencing homelessness.
- Ability to oversee complex funding and budgeting.
- Knowledge of local coordinated entry systems referral process.

Working Conditions and Physical Requirements
Must be willing to use vehicle for service provision throughout a seven county area. Works in close proximity to others staff residing at the VOA Mission.
