- Assures quality of care by adhering to Best Practice; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
- Consults and communicates care objectives with primary care providers, specialists and community resources to aid patient in achieving healthcare goals. Including and not limited to assessing the healthcare, education and psychosocial needs of the patient/family.
- Develops an individualized plan of care with targeted interventions, monitors patient and family’s response to plan of care and adjusts plan of care as needed. Provides patient with self-management tools/support to build ability for self-care.
- Follow up on patient’s hospital admissions/Emergency Department visits and MI Care visits.
- Documents patient care services by documenting in patient and department records.
- Examples: MI Care log, EMR and transportation log.
- Assigns proper billing/coding for services rendered.
- FHC Employee Engagement: participates in organizational events within the community and Family Health Center.
- Maintains professional and technical knowledge by attending educational workshops: monthly staff meetings, clinical committees.
- Adhere to the laws regarding safeguarding medical information according to Health Insurance Portability and Accountability Act (HIPAA).
Qualifications:
- Minimum 3-5 years current Case Management experience (INPATIENT OR CLINIC SETTING) - Required
- LMSW or LLMSW license preferred. But, will take LPC’s as well - REQUIRED
- Case Management experience specifically related to Substance Abuse - REQUIRED
- Phone Triage experience - Required
- Prior Authorization (Medication) experience - Required
- Adequate transportation and Valid Driver License - Required
- Community/Health/Public Health experience - Preferred
- BSN - Preferred
- Minimum 3-5 years current Case Management experience (INPATIENT OR CLINIC SETTING)
