Job Summary: The Stroke Coordinator will assist Saint Francis Hospital Muskogee in achieving Primary Stroke Designation through DNV. Primary Stroke Certification will ensure that Saint Francis Hospital Muskogee continues to provide high quality across the continuum of care as outlined in the Saint Francis Health System Planning Guidelines. Once certification is achieved, the Stroke Coordinator will play a vital role in continuing to monitor patient outcomes and implementing strategies for ongoing improvement.
Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom. Bachelor's of Science in Nursing preferred.
Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License. BLC, ACLS, NIHSS (National Institute of Health Stroke Scale Certification), or CCRN (Certified Critical Care Registered Nurse) preferred.
Knowledge, Skills and Abilities: Able to communicate clearly, both verbally and writing. Must be able to develop relationships with nursing and physician staff.
Essential Functions and Responsibilities: Responsible for assuring evidence-based standards of care for patients are met as required by DNV. Responsible for coordination of the quality assurance process, assessment, patient flow, and performance improvement. Also responsible for staff education. Responsible for community education and outreach. Responsible for working to assure that the SFHM program mirrors the SFH program. Follows patients with acute stroke during hospitalization, for the purpose of applying evidence-based practice to improve patient outcomes both physiological and financial. Ensures compliance of in-patient stroke indicators and maintains a treatment and outcomes stroke database. Acts as a clinical resource and manages the clinical management of stroke across the continuum of care for a significant patient population. Serves as the interdisciplinary team leader to provide coordination of the patient's care from pre-admission to post discharge.
