JOB SUMMARY:
The Staff Pharmacist contributes to therapeutic decision-making by participating in patient care, particularly pharmaceutical care, providing drug information, formal and informal presentations and consultations on medication use in the hospital. He/she assures the accurate and efficient distribution of drugs and oversees the usage of pharmaceuticals within the facility. He/she also promotes rational, cost-effective use of drugs within the facility while assuring the quality of pharmaceutical care and optimal clinical outcomes. In addition, the Staff Pharmacist provides authoritative drug information to other health care professionals, patients and the public.
JOB SPECIFICATIONS:
EDUCATION
Requires level of education necessary for licensure as a pharmacist by the California Board of Pharmacy. Prefer graduation from an accredited school of Pharmacy. Prefer Pharm.D. degree and/or completion of an ASHP accredited residency program or equivalent.
EXPERIENCE
Two years experience in an acute care hospital pharmacy preferred.
PREREQUISITE SKILLS
Current knowledge of drugs, their composition, actions, uses, toxic effect, dosage, availability and distribution. Ability to prepare sterile solutions. Familiarity with Federal and State laws regulating the profession of pharmacy and handling of drugs. Superior ability to work tactfully with patients, public, physicians, and other members of the healthcare team, and skillfully supervise daily workflow and personnel. Ability to demonstrate initiative, and problem identification and solving skills. Excellent written and verbal skills for teaching, presentations and persuasion. Demonstrated organizational skills. Knowledge of pathophysiology and disease states required to evaluate complex therapeutic issues and to monitor and advise on drug therapy.
LICENSE/REGISTRATION/CERTIFICATION:
Licensure as a pharmacist in the State of California required.
DUTIES AND RESPONSIBILITIES:
ESSENTIAL (not modifiable)
- Provision of clinical and distributive services consistent with patient focused pharmaceutical care.
- Daily monitoring of patients' therapies, including pharmacokinetic monitoring of patients for appropriateness of indication, dosage, duration, untoward effects and clinical progress.
- Intervenes when medication orders are unclear, inappropriate or sub-optimal.
- Enforces the closed drug formulary, evaluating use and need while suggesting alternatives and promoting cost-effective drug therapy.
- Participates in the instruction and supervision of externs, interns and residents.
- Provides clinically relevant drug information to physicians, nurses and patients.
- Performs patient discharge medication teaching as required.
- Ensures the accurate and efficient processing of orders.
- Engages in regular, continuous decision-making, impacting people, costs and quality of service.
- Participates in the identification and establishment of programs and procedures that ensure cost-effective drug therapy.
- Complies with applicable laws, regulations and standards of practice.
- Promotes a positive, supportive environment within the department and hospital by demonstrating a pleasant and helpful attitude.
- Oversees and participates in drug distribution process in inpatient, outpatient and IV additive areas of the pharmacy.
- Attends staff meetings, contributing to meeting agendas, discussions and problem resolution.
- Contributes, via presentation, to the Pharmacy & Therapeutics Committee.
- Directs workflow, including technical, billing and distributive functions, particularly in the absence of the immediate supervisors designated for these functions.
- Executes responsibilities within the established guidelines of hospital and departmental policy & procedures.
- Participates in department projects or committees.
- Participates in hospital committees as assigned.
- Ensures collection and assembly of departmental quality assurance data, as well as instruction within the department to improve or ensure threshold compliance.
- Participates in the development and execution of drug utilization reviews.
- Participates in the training and orientation of new employees as necessary.
- Identifies needs and provides education to pharmacy staff and other departments via informal and formal methods, such as in services and newsletters.
- Responds to Code Blues with appropriate participation.
- Regular attendance at interdisciplinary patient care meetings, and other applicable meetings, committees, etc.
SECONDARY (modifiable)
- Executing routine monthly floor stock inspections for nursing units.
- Other duties as assigned.
