AmeriMed Infusion Pharmacy in West Chester, Ohio is Now Hiring a PRN Staff Pharmacist.
AmeriMed provides home infusion therapy services to help patients safely transition care to their home, alleviating long and unnecessary stays in the hospital. Through our experienced and professional pharmacy team at AmeriMed we provide total care for our patients. Working directly with physicians, we are able to manage a variety of diseases and conditions in a homecare setting. Our goal is to help each patient be an active participant in their care. Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy.
Responsibilities
As a Pharmacist at AmeriMed Infusion Pharmacy, you are responsible for providing safe and appropriate pharmacy services to home infusion patients. As a Pharmacist at AmeriMed Infusion Pharmacy, you will access patients’ suitability for home infusion care, review and evaluate medication orders, labs and other clinically relevant information monitoring patients’ progress and making recommendations when needed.
Day-to-Day, your role includes:
- Assess patients’ suitability for home infusion care in accordance with Infusion’s policies and procedures. Knowledgeable of medical conditions and prescribed medication therapies suitable for home care services.
- Monitor and evaluate patient specific pharmacy care plans for the pharmacotherapy of home infusion patients. Collect and organize all patient related data needed to prevent, detect and resolve medication-related problems. Design and modify pharmacy care plans based on evaluated information and make recommendations for continued monitoring when appropriate.
- Receive medication orders from physician and evaluate the appropriateness of orders for dosage, drug choice, potential drug interactions, drug-food interactions, route of administration and stability.
- Ensure medication specific laboratory tests are ordered and results received. Responsible for the retrieval, review and consultation with physicians and nurses of ordered labs.
- Manage the use, maintenance and troubleshooting of all pharmacy equipment. Ability to accurately program, maintain and troubleshoot all IV medication related equipment.
- Provide medication-related education to Infusion staff, nurses, physicians, patients and caregivers.
- Assist the physician, nurse, patient, caregiver and Infusion staff with continued technical and clinical support. Provide accurate, concise and timely responses to clinical questions from health care providers, patients and caregivers.
Qualifications
- Active Ohio Pharmacist License.
- Experience with and knowledge of IV admixture procedures preferred.
- Excellent communication skills: oral and written.
- Flexible and able to work with a multidisciplinary team.
- Excellent time management skills and able to work independently.
- Ability to function well as part of team.
- Ability to work on call as needed including Weekends & Holidays.
Pay Range
$49.52 - $71.80 /hour
