Job Summary: This position is a Clinical Pharmacist, working with physicians, nurse practitioners, nurse managers, and other clinical staff at Heart of Ohio Family Health as part of a medical care team focusing on patient outcomes while maintaining cost. The Clinical Pharmacist provides patients and providers with therapeutic assessments and tailored disease management under a consult agreement, working toward improving patient and practice outcomes. The Clinical Pharmacist will also be the pharmacist support to the 340B program, assist with quality improvement initiatives, and assist facilitating group classes for chronic disease states.

ESSENTIAL FUNCTIONS:
- Assist provider as needed to meet Medical Home Certification (PCMH) and meaningful use guidelines
- Responsible as the 340B program Pharmacy expert
- Embedment in multiple practices across all sites as part of the medical care team
- Provide clinical pharmacy input during the preparation of interdisciplinary care plans for patients
- Assist in the selection of the most cost effective, evidence-based regimen for a particular patient; monitor patients for appropriate lab and other recommended follow-up
- Serve as a drug information resource for the providers and support staff
- Monitor patient response to treatment, drug/drug and drug/disease interactions; assess medications prescribed at dosages higher than approved by the FDA for specific indications
- Provide highly specialized education and training support to other professional services in the broad area of advanced pharmacotherapy, with emphasis on chronic disease states
- Utilize existing computer resources to maximize retrieval of clinical information for monitoring and review of the medication use process.
- Coordinate/participate in activities related to improving outcomes for targeted disease management initiatives leading to improved health as listed in the consult agreement (such as COPD, Asthma, Hypertension, Diabetes, etc.).
- Identify patient/family gaps in knowledge of diagnosis and or treatment and development interventions with patient/family input to mitigate gaps.
- Coordinate referrals with mental health care managers, etc. as needed.
- Make referrals to the appropriate Medicaid experts for Medicaid contractual and coverage issues.
- Priority Patient Populations: Medicaid, Dually eligible, Medicare, Uninsured, Commercial Insurance.
- Coordinate and support pharmacy initiatives, such as 340B program, Over the Counter (OTC), Standing Orders, Preferred Drug List (PDL), and electronic prescribing (e-prescribing) efforts, as outlines by the Ohio Board of Pharmacy.
- Assist physicians and nurse practitioners in creating and managing the drug regimens of patients with chronic disease states (e.g. diabetes, asthma, hypertension, etc.). This may include, but shall not be limited to, activities such as meeting with patients, adjusting medication dosages per the consult agreement and performing other services within the professional area of expertise.
- Perform medicine reconciliation assessments as requested by Heart of Ohio Family Health providers and/or case managers to optimize the patient’s drug regimen.
- Serve as a resource to Heart of Ohio Family Health providers and case managers on general drug information, 340B program, and Medicaid drug policy issues.
- Other duties as assigned.
Education Requirements:
- Degree (Pharm.D., preferred) from an accredited school of pharmacy.
- Current pharmacy license in the State of Ohio.
- Completion of residency, fellowship, and/or 1-2 years of applicable experience (preferred but not required).
- Knowledge of insurance and third-party payment systems and 340B experience (preferred but not required).
Facility Environment:
All facilities have a medical office environment with front-desk reception area, separate patient examination rooms, nursing stations, pharmacy stock room, business offices, hallways and private toilet facilities. All facilities are on the main ground floor and ADA compliant.
This position’s primary work area is the pharmacist’s office, patient examination rooms, and nursing stations. The patient examination rooms and nursing stations areas are:
- kept at a normal working temperature
- sanitized daily
- maintains standard office environment furniture with adjustable chairs
- maintains standard office equipment; i.e., computer, copier, fax machine, etc. at a normal working height
