Monday thru Friday 8:00a.m. - 5:00p.m. with one-hour break. Other hours and/or days as needed for program activities.
Qualifications: The Medical Case Manager must have a degree in human or social services, health education, or a related field.
Must have experience in HIV/AIDS care, along with excellent interpersonal skills and the ability to establish the confidence and cooperation of patients and families from varied backgrounds. Must also have excellent oral and written communications skills and project a positive image in the community.
Job duties include but are not limited to:
- Work with primary care providers and clients to carry out treatment plans. Provide on and offsite rapid testing as needed. Assess clients for substance abuse. Provide patients with supplements on an as needed basis. Provide additional counseling and/referrals as needed by patients.
- Develop adherence treatment plan with client identifying ways to overcome client's barriers.
- Provide education to community agencies, schools and churches regarding the need for early intervention and early enrollment in care as well as prevention education as needed. Participate in staff development sessions.
- Input necessary data in Provide Enterprise data system to monitor and generate reports.
- Assist in coordinating client(s) in need of additional or intensive following routinely staff client issues with support staff to ensure continuity of care. Serve as guidance for other personnel working in the capacity of care coordination.
- Keep case management files in complete compliance with the SC Ryan White CM Guidelines (i.e., assessments, reassessments, service plans, etc.) in the first year and sustain thereafter.
- Participate in planning and implementation of quality assurance projects.
- Complete comprehensive psychosocial assessments with people living with HIV/AIDS (PLWHA) to identify medical and medication needs, provide appropriate referrals, and assist clients with completion and submission of governmental benefit applications as needed.
- Coordination of service planning and follow-up through treatment adherence, medication and medical care adherence with frequent client contact to assess service needs.
- Provide education and advocacy for PLWHA clients in the following areas: medical care and medication adherence, risk reduction, oral health, nutrition, mental health, and substance use.
- Provide applicable linkage and referrals to community services and programs.
