Haymarket Center is the largest not-for-profit community-based adult detoxification, residential, and outpatient substance abuse treatment facility in Chicago. Haymarket Center has continued to grow into a comprehensive alcohol and other drug treatment organization, licensed by the state of Illinois, which receives funding from the private sector, as well as city, county, state and federal agencies. The treatment programs are accredited by the Commission on Accreditation for Rehabilitative Facilities (CARF).
Provide Medical Case management services under AIDS Foundation Chicago's Co-operative Case Management system to community-based clients living with HIV/AIDS who have recently exited the corrections system. Provide a range of client centered, confidential services that link clients with core including legal, medical transportation, emergency financialassisance, food and housing. The goal of Medical Case management is to ensure viral suppression by taking care of barriers to care services preventing a client from being undectable. The Medical Case Manager will provide treatment adherence counseling to endure readiness for, and adherence counseling to ensure readiness, and adherence to complex HIV/AIDS treatments.
ESSENTIAL FUNCTIONS:
Carries a caseload assigned through AIDS Foundation Chicago (AFC).
- Follow all protocols established AFC and if applicable, the IDPH;
- Conduct outreach services and field visits as needed to patients who have disengaged from care;
- Complete emergency financial assistnce and payments requests as needed for copays;
- Develop inital patient service plans with patient input;
- Update patient service plan with patient, family and physician input as required;
- Maintain advocate position for patient position through service episode;
- Provide necessary documentation in patient chart;
- Facilitate therapeutic groups when required;
- Utilize established outreach and safety procedures for improved patient contact;
- Develop patient resources;
- Participate in appropriate networking and public relations activities;
- Atend required internal and external meetings and inhouse trainings as needed;
- Complete all documentation and reports on a timely basis;
- Other duties as required for effective team work or as assigned by Supervisor.
Requirements
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
- Previous experience working with substance abuse & HIV patients required;
- Knowledge and/or experience with corrections population highly desired;
- B.S. degree in social sciences or related field;
- Knowledge of "Core Functions".
- Experience in substance abuse, HIV or related field.
- Valid Illinois Driver's License and good driving record required;
- General computer literacy mandatory;
- Good communication skills in oral and written form;
- Previous CM experience beneficial;
- Ability to work independently and as part of a team.
