Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will be responsible for providing support and assistance to individuals and families who are dealing with various challenges. Your role will involve assessing clients' needs, creating care plans, coordinating services, and advocating for their well-being. Qualifications:
- Bachelor's or Master's degree in Social Work or a related field.
- Valid state licensure or certification as a Social Worker (LCSW, LMSW, etc.).
- Proven experience working as a Social Worker, preferably in a healthcare or community setting.
- Sound knowledge of social work principles, ethics, and best practices.
- Excellent communication and interpersonal skills.
- Strong problem-solving and critical thinking abilities.
- Ability to maintain confidentiality and exercise discretion.
- Empathetic and nonjudgmental approach towards diverse populations
- Long term care experience preferred
Responsibilities:
- Conduct comprehensive assessments of clients' needs, including social, emotional, and environmental factors.
- Develop and implement individualized care plans in collaboration with clients and their support system.
- Connect clients with appropriate community resources, programs, and services.
- Advocate for clients' rights and work towards improving their quality of life.
- Provide counseling, emotional support, and crisis intervention to individuals and families.
- Evaluate the effectiveness of interventions and modify care plans as necessary.
- Collaborate with healthcare professionals, agencies, and organizations to ensure coordinated care.
- Maintain accurate and up-to-date client records and documentation.
- Stay current with changes in social work policies, regulations, and trends.
