The Social Worker is someone that represents the interests of the residents and the families of St. Margaret's. This is the primary person responsible for the psycho-social well being of the elders that live here with us. This person is an unbiased lesion with understanding of the residents’ rights and basic community resources. This person should also be able to problem solve and manage grievances and requests. This person works with all departments in the nursing home to maintain a happy resident population.
- Monitor through interviews and floor rounds, “Dignity Issues,” for residents nail care, hair care, clothing repair, state of rooms, staff resident interaction etc.
- Assessments that correspond to Medicaid and Medicare billing
- Participate in care plan conferences
- Manage resident requests: Popular topics include; cable issues, phone issues, store runs for special supplies, IOP, change of PCP, roommate issues ,lost/ stolen items.
- Chart Documentation
- Investigate Incident Accidents and participate
- Discharge Planning
- Management of Community Resources
- Work to anticipate where an issue may arise and attempt to alleviate this through coordinating family, elder and care teams.
- Grievance Log upkeep
- work through discharge planning for anyone returning to the community
- Manage the Medicaid application process
- Manage and assist with enrolling and disenrolling of insurances
Job Type: Full-time
Pay: $45,000.00 - $55,000.00 per year
