SalaryUnder direct supervision of the Human Services Supervisor, this position supports the mission of Blue Earth County Human Services by effectively and efficiently delivering essential services to families and individuals so they can achieve their potential through active community participation. The Social Worker – Supportive Housing position is responsible for providing general and targeted case management (TCM) services to eligible households, including single adults and families who have experienced housing instability and homelessness. The households may also experience physical, mental, and/or chemical health concerns. This position is responsible for completing Supportive Housing program eligibility determination and intake duties, developing individual / family community support plans, providing case coordination services, coordinates services provided by outside agencies, provides ongoing assessment of services to households ensuring they are appropriate and effective, provides ongoing assessment of local care systems to assure needed resources are available, maintains required documentation and written records, completes activities and records to maximize revenue reimbursements, and participates in professional development activities.

Pay: $32.91 - $46.12 per hour
