Title: Program Specialist (Social Worker)

Location: REMOTE to FL (REGION 8-Lee County Areas Lehigh, Alva, Labelle)

*Making face-to-face visits at members' homes or other agreed upon public locale

Duration: 3 months contract to hire

Target Start Date (based on successful onboarding completion): 05/06/2024

Shift: 8:00 AM - 5:00 PM - Monday - Friday - No OT

Walk me through the day-to-day responsibilities of this role and a description of the project:

• COORDINATING CARE FOR SPECIAL NEEDS FAMILY ON THE CMS PLAN
• Works on non-acute members

• CASELOAD COMPLIANCE PER CMS CONTRACT. FIELD VISITS, ASSESSMENTS, CARE PLANS

Required Skills/Experience/Education:

• 1+ years Pediatric Experience & Master of Social Work Degree MSW or LPN with 4+ years Pediatric Experience
• Experience with electronic medical records & Computer Literacy - Microsoft Office (Excel, Outlook, Word)
• Prior home health care, hospital, care management experience. Accountability, prioritizing, clinical judgment, team player, ability to take direction, and self-discipline sufficient

Job Type: Contract
