Location: Columbia, Missouri 
Shift:  Shifts will be for day shift any day of the week (Sunday through Saturday) 

Department: MUPC Case Management 
Salary Range: PRN rate is $25 per hour

Job Summary: 

Assesses patient and family psycho-social and discharge planning issues relevant to medical treatment. Provides crisis intervention, emotional support, resource information, discharge planning, legal reporting and facilitates ethical consultations.  

Essential Functions

  1. Assess needs and locate community resources for patients and families in need of concrete services such as transportation, lodging and food; Develop/maintain effective working relationships with community agencies. 

  2. Assist patients/families with issues requiring intervention within the hospital and community. 

  3. Consult and collaborate with health care providers and other staff regarding patient care, problems and discharge planning. 

  4. Counsel, plan and implement discharge and post-hospital care with patients and families considering age specific needs. 

  5. Document social services provided appropriately and in accordance with department/hospital policy and maintain department statistical records. Participate in department and hospital committees as assigned. 

  6. Interview patients and families for social information relevant to treatment and health care planning and compile comprehensive social evaluations. 

  7. Provide crisis intervention, emotional support, counsel for adjustment to illness/disability for personal/family problems related to health/illness on bio-psychosocial issues and complex situations. Refer patients and families to community, state and federal agencies and resources and provide coordinative services. Perform legal reports for suspected child abuse, elder abuse, domestic violence, and guardianship referrals. 

  8. May complete unit/department specific duties and expectations as outlined in department documents.  

  9. Interview patients/families for social information relevant to health care planning in order to establish a treatment program; counsel patients and families for adjustment to illness/disability in situations of limited complexity. 

Required Qualifications: 

  • Masters of Social Work degree. 

Preferred Qualifications: 

  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) licensure by the Missouri State Committee for Social Workers board. 

  • Additional license/certification requirements as determined by the hiring department.

This position is very active and may require ambulating, remaining in a stationary position and positioning self to reach and/or move objects above the shoulders and below the knees. The employee may frequently lift and/or move objects up to 10 lbs. and occasionally move objects up to 25 lbs.
