We currently have an opening for a Per DiemSocial Worker in Anaheim. This is a field role.
The Per DiemSocial Worker (MSW) is the member of the interdisciplinary team whose primary function is to provide diagnostic assessments of the psycho-social and economic needs of the patient/family unit, define the social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.
PRIMARY JOB DUTIES AND RESPONSIBILITIES:
- Participates in the preparation, evaluation and implementation of the interdisciplinary plan of care to assure that the highest quality of social services is offered to patients/families and that the goals of continuity and comprehensiveness of care are achieved.
- Provides psychosocial and emotional support to the patients/families.
- Maintains appropriate documentation in the hospice patient chart.
- Attends Interdisciplinary Team (IDT) meetings for case review and development of plan of care, assists IDT in understanding the significant social and emotional factors relating to a terminal illness.
- Participates in in-service education programs.
- Facilitates appropriate community resources and provides casework.
- Act as a resource for other IDT members of the interdisciplinary team at the direction of the team manager. Completes assessment timely per policy.
- Perform other duties as assigned.
Qualification: (Education, experience, knowledge, skills and abilities required to perform duties/responsibilities.)
- Master of Social Worker Degree (MSW) from a school accredited by the Council on Social Work Education and clinical experience relevant to the psycho social needs of patients and families. Licensure if Licensed Clinical Social Worker (LCSW).
- One (1) year of social work experience in a health care setting.
- Knowledge of stress of repeated loss as well as to provide support to the rest of the team. Knowledge of community resources and the ability to utilize these resources. Knowledge of the end of life process and bereavement. Knowledge of non-invasive pain and symptom control.
- Demonstrates the ability to use good listening and communication skills.
- Able to multi-task effectively.
- Capacity to relate and work as a member of an IDT.
- Current driver's license and proof of insurance required.
- BLS required. (Can be obtained once hired)
- Traveling required (Field position)
- Flexible schedule as availability is needed based on patient needs
