The Social Worker plays an integral role on the interdisciplinary team at St. Francis Hospital.
Essential Responsibilities and Duties:
- Conduct psychosocial assessment and provide ongoing counseling to patients that are referred from the medical team or that are identified as experiencing emotional distress through Psychosocial Distress Screening.
- Collaborate with Interdisciplinary Team on an ongoing basis to ensure that the medical treatment plan is being followed, and the most optimum outcome is reached.
- Document in the EMR and in MIDAS.
- Record statistics of community calls received and interventions provided.
- Attend conferences and in-service trainings to maintain competencies and keep current with available community resources.
Educational Requirements
- Master's Degree in Social Work from an accredited School of Social Work.
- Current New York State Social Work license.
The rate for this position is $45.00 per hour.
