PURPOSE OF THE POSITION:
The position serves youth with special needs and their families through the Coordinated Services Teams (CST) program in both Pepin and Buffalo counties. Position also works with individuals across the lifespan with mental health and/or substance abuse diagnoses through a wraparound approach in the Comprehensive Community Services (CCS) program. Position backs-up the crisis program and serves individuals under Chapter 51 mental health commitment orders. Position is also part of the on-call and access team.
DUTIES AND RESPONSIBILITIES:
- Fulfills requirements of Service Facilitator for the Comprehensive Community Services (CCS) program as outlined in Wisconsin Administrative Code DHS 36.
- Fulfills requirements of Service Coordinator for the Coordinated Services Team (CST) program as outlined in Wisconsin Administrative Code DHS 46.56.
- Provides services through wraparound/team-based approach in support of recovery concepts and practices.
- Conducts comprehensive, strength-based assessments through interviews, home visits and collecting of collateral information.
- Assesses ongoing consumer needs while developing and updating service plans to address needs; coordinates and monitors ongoing delivery of services to consumers.
- Explains the scope of services and discusses consumer’s rights and responsibilities in relation to the use of services.
- Coordinates and participates in recovery team meetings with the goal of bringing teams together to work collaboratively with and for consumers.
- Participates in the CST Coordinating Committee meetings and completes activities to support the committee and program.
- Collaborate with the coordinating committee, area schools and agencies to support CST and the Interagency Agreement.
- Facilitates public education and awareness of issues and programs for children/youth who are involved in two or more systems of care.
- Provides community services per Wisconsin statutes, court policies, and/or requirements of program, state, or federal guidelines.
- Maintains documentation that meets professional practice, billing expectations, and program standards.
- Prepares correspondence, reports, and records as necessary and appropriate.
- Provides and complies with all crisis program services requirements.
- Provides services to individuals referred to the department under Chapter 51.
- Collaborates with Corporation Counsel to meet court requirements and processes for individuals under Chapter 51 commitment orders.
- Testifies in court as necessary or required.
- Adheres to approved Social Worker principles, methods and practices, including the National Association of Social Workers (NASW) code of ethics.
- Provides access duties on a rotating basis.
- Provides on-call duties after hours on a rotating basis in compliance with 48.067 and 938.067.
- Provides mental health mobile crisis assessments after hours on a rotating basis.
- Exercises powers and performs intake duties under WI Statutes 48.067 and 938.067, to include taking juveniles and children into custody under WI Statute 48.19, 48.193, 48.195 and 938.19, 938.355 (6d).

- Under direction of the supervisor, this position may at times be required to work overtime hours or shifts.
NON-ESSENTIAL FUNCTIONS:

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
- Bachelor’s degree in social work or closely related human services field
- Social Work Certification or commitment to enroll in an accredited educational program within one year of hire and obtain Social Work Certification upon completion
- Three years’ experience with individuals or families with mental health, substance abuse, and/or abuse and neglect issues preferred
- Successful completion of the Juvenile Justice Intake training
- Good written and oral communication skills; tact and courtesy; thoroughness and dependability
- Knowledge of the functions, procedures, organization, policies, and regulations governing the administration of the Comprehensive Community Services Program
- Knowledge of and ability to provide services to individuals across the lifespan in a wraparound approach
- Ability to maintain confidentiality in dealing with all pertinent information
- Ability to establish and maintain effective working relationships with peers and supervisors
- Proficient with Microsoft Office products and the ability to quickly become proficient with other required software.
- Valid motor vehicle operator’s license and/or available means of transportation required.
- Available to be on call as assigned
