Job Description
St. Elizabeth Nursing Home, an Illuminus Community, is seeking a highly organized and compassionate individual to join our team as full-time Referral Specialist/Life Coach (Social Worker). In this role, you'll be responsible for maintaining census goals through managing referrals, coordinating admissions and resident transitions as well as discharge planning, facilitating adjustment to nursing home placement with residents and their families, and maintaining appropriate records accordingly.

Originally founded in 1967, St. Elizabeth is located just blocks from downtown Janesville in the beautiful Courthouse Hill Historic District between Upper Courthouse Park and Jefferson Park.

Responsibilities
- Maintains and actively promotes effective communication with all individuals.
- Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
- Receives and responds to referrals and/or inquiries swiftly with a desire to convert the referral into an admission.
- Reviews and identifies clinical and equipment needs of prospective residents with the interdisciplinary team to ensure readiness for admission.
- Communicates and collaborates effectively with referral sources.
- Coordinate referrals and process admissions including welcoming and on-boarding newly admitted residents and facilitating adjustment to nursing home placement with residents and their families.
- Assist in obtaining hospital documentation necessary to complete Minimum Data Sets.
- Create contacts that enhance development of business partnerships.
- Maintains and actively promotes effective communication with residents/patients/clients and family members / personal representatives.
- Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
- Schedules and facilitates quarterly care plan reviews. Provides interdisciplinary staff with appropriate information. Reviews information on the overall plan of care and records appropriate information on the same. Completes MDS as assigned.
- Assists in completing a social history on a resident within two weeks of his or her admission. The social history shall include identifying information, previous life experiences, significant inter-personal relationships, family relationships, illnesses, significant events, and any other interest which may be used by the inter disciplinary team in coordinating the overall plan of care.
- Maintains resident and family contact, especially when special problems/needs arise.
- Assists residents/responsible party in completing and signing necessary forms, including Advance Directives.
- Facilitates resident and family sharing of concerns or questions.
- Recommends referrals to appropriate agencies in cases of financial or psycho-social problems or community discharge.
- Assists residents and their support system with discharge planning, as appropriate.
- Compiles information about community health and social services available and keeps a current resource file on these services.
- Accepts and investigates all formal and written grievances submitted, as well as informal complaints, and then consults with the Social Worker Supervisor and/or the Administrator or DON in the timeliest manner appropriate. Maintains a file containing grievance forms and completed, resolved grievance issues.
- Responsible for on-call services, as assigned.
- Performs other duties, as assigned.
Qualifications:

- Ability to adapt to change.
Experience & Training:
- Bachelor in Social Work or related field.
- Social Work Certification/License
- Prefer 6-7 years’ experience in sales, marketing or related field.
- Understanding of the Acute Care and Post-Acute Care Transition Cycles.
