What is PACE North? PACE stands for Program of All Inclusive Care for the Elderly. We are new and unique option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly in daily both our state-of-the-art day center on Garfield near Hammond during regular business hours and as needed in their home. Never heard of us before? That’s okay, we’re new to the region. We began serving the elderly in 2019 and our service model is so popular it’s growing quickly!

What does a Social Worker at PACE North do? You can expect to be responsible for direct social work case management services for our participants, provide social services support as, participant screening, case management, referrals, and counseling. Participates in our Interdisciplinary Team assessments, care planning, and re-assessments as needed. Traveling to our participants homes for site visits when needed, active participation in Q1 plan processing, participates in Program and Policy development of Social Work Component.
Requirements:What qualifications should you bring to the table? We’re looking for that special person who loves serving others with a positive approach. We’d appreciate at least 1 experience in an elderly patient setting, either a bachelor’s or master’s degree from an accredited School of Social Work, have a CPR certification (we can train CPR if needed!), have a valid driver’s license, be medically cleared for communicable diseases, and have recommended immunization up to date before engaging in direct contact with participants. With the driver’s license we also ask for valid auto insurance and reliable transportation.
