Social Worker - Behavioral Health

MUST:
Experienced Social Worker
Minimum of 1 years direct services experience as a Social Worker
Must hold a masters degree in in mental health
Must be a Licensed Clinical Social Worker ( LCSW ) or Licensed clinical professional ( LCPC ) counselor in Maryland
Must be bilingual in Spanish and English
DUTIES:
The Social Worker - Behavioral Health will be responsible for but not limited to the following:
Develop and maintain client case records, plans and reports
Provides social work and discharge planning services including assessment, planning, and intervention to patients and their families
Develops effective working relationship with patients and their families
Compile data for statistical and billing purposes.
Attend and participate in stabilization meetings, as needed
Conduct intake evaluations
