Job description

Under general supervision of the Human Services Supervisor - Behavioral Health, this position supports the mission of Blue Earth County Human Services by effectively and efficiently delivering essential services to families and individuals so they can achieve their potential through active community participation. The Social Worker - Behavioral Health Case Manager will be responsible for providing Adult Mental Health Targeted Case Management (TCM) Services to adults with serious and persistent mental illness and to adults under court commitment as mentally ill or chemically dependent. This position is responsible for assessing, accessing, referring, coordinating, monitoring and planning mental health services. This position also monitors court commitments, participates in discharge planning, provides crisis assistance and participates in professional staff development.
ESSENTIAL FUNCTIONS:

  1. Provides Adult Mental Health Targeted Case Management Services in accordance with Minnesota Statutory requirements and Department of Human Services guidelines. Meets with clients to assess strengths, service needs, and available community supports. Develops person-centered case plan and goals. Coordinates and monitors the provision of services between clients and service providers to assure services are being provided, utilized, and effectively meet the needs of clients. Provides case management services including assessment, planning, referral/linkage to community-based services and monitoring/service coordination. Opens, maintains, and closes case files. Case management services may include helping the client obtain needed mental health services, social services, educational services, health services, vocational services, recreational services, and related services in areas such as housing services, advocacy, transportation, and legal services. Travels to appointments within and outside of Blue Earth County.
  2. Develops and revises Functional Assessments (FA), Individual Community Support Plans (ICSP), and General Service Plans. Identifies service needs and develops goals with the assistance of the client, family members, service providers, and other significant parties. Reviews ICSP, FA, and General Service Plan and consults with clinical and unit supervisors to assure that goals and services are appropriate to the needs of the client.
  3. Completes documentation and reports. Maintains an ongoing written record of all contacts with client and collateral sources. Assists with scheduling diagnostic assessments and updates as determined by rules governing service delivery. Monitors and reports daily activities, recording time spent with clients as well as non-billable activities. Reviews client files and related correspondence. Completes or updates necessary data privacy and consent for release of information forms.
  4. Monitors court commitments and compliance of mental health and/or chemical health services and treatments ordered by the Court. Writes reports to the court as mandated by statute.
  5. Participates in discharge planning by coordinating with clients and facility staff to identify support needs, develop discharge goals/plans, and facilitate referrals and appointments.
  6. Provides crisis assistance by identifying and accessing additional support services during times of mental health crisis.
  7. Promotes a diverse, culturally competent, and respectful workplace.
  8. Interacts Positively with Internal and External Customers. Communicates and interacts effectively and respectfully with employees, supervisors, individuals from other organizations, and customers.
  9. Performs the Physical and Mental Demands of the Position.
  10. Attends work during regularly scheduled hours.

ADDITIONAL FUNCTIONS:

  1. Coordinates and monitors CADI Waiver Services and refers clients to appropriate services when applicable.
  2. Performs intake duties as assigned by obtaining information from clients to determine service needs. Assists clients to obtain information about local resources and provides referrals as needed.
  3. Assists with completing pre-petition screening for court commitment as needed.
  4. Participates in professional staff development. Participates in training and education opportunities to maintain and improve proficiency. Attends and participates in professional organizations. Associates with agents and departments from other counties and states for collaboration.
  5. Performs Other Related Functions as Apparent or Assigned.
MINIMUM QUALIFICATIONS:
  • Bachelor's degree in social work, sociology, psychology, or related field from an accredited college or university; and
  • 2,000 hours of supervised experience in the delivery of services to adults with mental illness; or
  • Equivalent combination of education and experience necessary to perform the essential functions of the position.
PREFERRED QUALIFICATIONS:
  • 2- years' experience providing Adult Mental Health Targeted Case Management Services in a county human services agency.
  • Fluent in understanding and speaking languages other than English such as: Spanish, Somali, Chinese, Sudanese, or Arabic.
CONDITIONS OF EMPLOYMENT:
  • Must possess a valid driver's license and be able to drive in the State of Minnesota.
  • Must successfully pass a background check.
  • Must comply with all federal and state laws and organizational and departmental policies and regulations.
  • Must comply with data practices policies and standards relative to not public data. Any access to not public data should be strictly limited to accessing the data that are necessary to fulfill the employment responsibility. While data are being accessed, incumbent should take reasonable measures to ensure the not public data are not accessed by individuals without a work reason. Once the work reason to access the data is reasonably finished the not public data must be properly stored according to county policy and the Minnesota Statutes.
Monday through Friday from 8:00 a.m. to 5:00 p.m. This position allows for flexible scheduling and may require evening and weekend hours.
