RESPONSIBILITIES:
- Ensures resident adjustment to rehabilitation or long-term care by performing cognitve testing, initial plan of care, care planning and completing all necessary paperwork including MDS.
- Reviews Resident Rights with all new admissions.
- Ensures proper paperwork is completed.
- Schedules interdisciplinary team meetings with medical staff, residents and family members in order to plan care and resolve concerns. Write reports and ensures implementation.
- Demonstrate understanding and provide guidance and counseling to residents and their families on diverse issues ranging from grief and loss to acceptance and coping with changes in living arrangements.
- Coordinating discharge services - planning and acquisition of appropriate follow up care
- Ability to listen and act compassionately with residents/family members; must be able to work on own and maintain job related jobs
- Document residents’ behavioral and psycho-social status
QUALIFICATIONS:
- Long Term Experience: 1-2 years minimum
- Bachelor's Degree in Social Work, Psychology or Related Field
