Job Description
I. Required Qualifications
A Degree in Social Work and a current license with the State of South Carolina required or a Degree in Social Work and must obtain licensure within 6 months (LMSW or LBSW). One year of related social work experience is required.
II. Preferred Qualifications
Experience in a healthcare setting.
III. Work Environment
Primarily works in the Emergency Department. Must be able to handle emergency or crisis situations up to 70% of the time and demonstrate emotional stability. Subject to long and irregular hours. Occasional standing and walking. Must be able to handle varying or unpredictable situations.

VI. Essential Functions
a. Screen patients for risk status
b. Collaborate with ED physician, inpatient Care Coordination staff, and inpatient physician on plan of care to prevent any avoidable readmissions
c. Define appropriate placement or community-based services to support the patient and prevent admission or re-admission
i. Primary care or specialty appointment
ii. Support services in the home (preferably)
iii. Placement in skilled nursing facilities
iv. Financial assistance, transportation, medications, community resources
d. Initiate proactive case finding, active rounding with charge nurse/physician and staff – identifying patients who can be supported at home rather than admitted
i. Psychosocial assessment
ii. Child, adult, domestic or elder abuse or neglect
iii. Crisis intervention
iv. Behavioral health concerns
v. Placement or acute outpatient interventions
vi. Works with patients, families and staff to access resources
vii. Works with law enforcement and Department of Social Services as appropriate
viii. Initiates and/or contributes to the plan of care
e. In addition to the above, the Social Worker proactively case finds and accepts referrals from the Care Coordination partner and ED staff and physicians
f. Initiate comprehensive care plans addressing patient’s total health needs, and/or contribute to and update established care plan
g. Communicate the plan of care and next steps with Care Coordination staff prior to patient’s admission to an inpatient bed
h. Provide education to ED staff and physicians regarding post -cute care, regulations and other appropriate content
i. Network with community providers and services
j. Adhere to the Care Coordination’s Transition of Care Standards
k. Performs other miscellaneous and related duties as required.
VII. Physical Requirements
Physical requirements are primarily related to the essential functions of any job. In order to perform the essential functions of this position, you must be able to stand, walk sit, use hands, stoop, talk, and hear. Must be able to lift or exert energy up to 10 pounds 100% of the time. Must have close vision (clear vision at 20 inches or less), and distance vision (clear vision at 20 feet or more).
IX. Mental Requirements
Must possess the ability to read and interpret business or safety manuals, clinical or technical procedures or regulations. Ability to write reports, correspondence and/or manuals. Must have the ability to effectively present information and respond to questions from groups, patients, families, other employees, and medical staff. Must have the ability to perform more complex math functions to include calculating percentages, averages, volume, ratio and interpreting graphs-using numbers, money and units of measurement. Must have the ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to deal with concrete and abstract concepts and interpret verbal, nonverbal and written instructions.
X. Population Served
This position primarily interacts with children, adolescents, adults and aging adults. All staff members must be able to demonstrate the knowledge and skill necessary to meet the physical, psycho-social, educational, safety, and related needs of the persons in the assigned work area.
