Job Description - SOAR Case Manager
POSITION OVERVIEW
The Diversion Hub is a comprehensive, one-stop network dedicated to assisting justice-involved individuals in Oklahoma City by harnessing the power of multiple support agencies through combined and synchronized services. Our goal is to provide life-stabilizing resources while empowering individuals to reduce their encounters with the criminal legal system through enhanced support services, including case management, data integration, and coordinated communication under one roof. SSI/SSDI Outreach, Access, and Recovery (SOAR) is a model that helps individuals experiencing or at risk of homelessness who have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder apply for Social Security disability benefits. The Diversion Hub SOAR Case Manager will provide direct case management services to clients and act as the primary point of contact and liaison between the client, Justice Navigator, and the service navigators.
RESPONSIBILITIES
Responsibilities of the SOAR Case Manager include but are not limited to the following:
- Demonstrating the values expected within the program, and for promoting the mission of the organization.
- Work in the community and meet individuals experiencing homelessness, who have a serious mental illness, and/or co-occurring substance use disorder; meet with individuals where they are, including in the woods, under bridges, and in abandoned houses.
- Stay organized and have an ability to prioritize tasks quickly.
- Attend any trainings/certifications as required by the state.
- Work with referral sources and community partners to identify applicants through team meetings, outreach, and referrals
- Complete interviews with individuals to gather information to complete SSI/SSDI applications
- Gather medical records and other information to complete SSI/SSDI applications
- Write SOAR Medical Summary Reports for individual applications
- Accompany individuals to appointments at the Social Security Administration
- Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application
- Coordinate case management services with partners and help with providing case management services to individuals when needed
- Always adhering to agency code of ethics and professional standards including maintaining strict client confidentiality.
- Attending all staff development, trainings and meetings as required.
- Any other duties within the scope, spirit, and purpose of the job and/or organization.
EDUCATION AND EXPERIENCE
- 1 to 3+ years of experience in social work, communication, criminal justice, political science, or similar human service area.
- Certification as a Peer Specialist (optional)
ADDITIONAL JOB REQUIREMENTS
- Clearance of background check.
- Must have own transportation for site visits. Valid driver's license and insurance on motor vehicle in accordance with state law and agency requirements.

COMPUTER EQUIPMENT AND SOFTWARE REQUIREMENTS
Intermediate computer, mobile device and internet skills with knowledge of software programs including but not limited to:
- Windows and Microsoft Office Products
- Cloud-based File Sharing such as Google Drive and SharePoint
- Adobe Products
- Client Databases
PHYSICAL DEMANDS AND WORK CONDITIONS
While performing the duties of this job, the employee must be able to sit over long periods of time and be able to talk, hear, sit, stand, walk; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; and stoop, kneel, crouch, or crawl climb stairs, lift and or move items of up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. Must have excellent command of English language and grammar, both verbal and written. Must be able to manually operate and use a computer. Must be able to clearly hear and understand telephone conversations. Must be able to travel for the purpose of meeting with clients, training, collaborating with stakeholders, or offsite personnel/management.
